15
You are receiving this e-mail communication because our records reflect that you have requested, or are currently on, an intermittent FMLA Leave of Absence.
16

17
As previously communicated to all SHPS associates, effective January 01, 2011 SHPS began utilizing Matrix Absence Management for our FMLA administration needs.
18

19
As such, going forward the next time you report an absence due to an intermittent FMLA leave, you will need to utilize the attached Time Tracking Sheet to report your absence to Matrix. A Leave Coordinator from Matrix will then contact you to review your existing Health Care Provider Certification and will ask you to provide updated information regarding your leave of absence. If you no longer need intermittent leave, please notify Matrix at the number provided below.
20

21
If you still need intermittent leave, each pay period you will still be required to submit a timesheet indicating the time you have taken toward your intermittent leave for that pay period. The timesheet is to be completed, signed by your supervisor and faxed back to Matrix. Matrix will then notify SHPS of the time you have requested.
22

23
Please make multiple copies of the attached Time Tracking Sheet, as this will need to be used each time you report intermittent time to Matrix.
24

25
You may contact Matrix at the address or telephone number listed below:

Hi Cathy -
42

43
I did see my MD this afternoon, and he stated I cannot return to work for 6-8 weeks. He said he cannot have the FMLA certification filled out today - that they hope to fax it to SHPS by Friday (to (502) 297-5345 which is the fax number on the form I have - please let me know if this number has changed) if possible, although he also stated per Federal Law they have 30 days to complete this form. He also stated per the FMLA form from SHPS I have 15 calendar days from the time I'm given the form from HR. I did have a copy of the blank form here at home & used this one today (although Mark Tatum did offer to email it to me). The right foot is fractured and the left ankle is sprained.
44

45
I also spoke with Mark Tatum a little while ago and asked if I am required to call in each day until the FMLA was approved/denied by Matrix, and if I understood correctly I was to direct this question to you. Is this necessary? I do not wish, after all, to create more work or incovenience than necessary for Marshall or Workforce.
46

47
My MD said to give you his information - if you have any questions feel free to call him:

Cathy, do I need to call in my absence to Workforce each morning 60 minutes prior to the start of my shift since my FMLA forms are currently being completed by my MD and I've notified SHPS that I will be out for the next few weeks for medical reasons?

127
128
129
130
Thank you, Mark. I am only asking because as you can see below I was told to call in every day until the FMLA was approved by Matrix. I do appreciate the clarification. I'd sent these from my SHPS email as my personal emails were not being responded to.
131
132
133
134
I have made silly errors in the past (like leaving when I could neither hear or breathe) that nearly cost me my job - I am simply trying to avoid that now and attempting to ensure I have covered all my bases on this end since my past experiences in working with my immediate supervisors have not been successful. So forgive me if I tend to stay on top of things to ensure everything is done correctly.
135
136
137
138
Just as with the problems I'd had with difficulty in hearing my calls at work. Cathy repeatedly told me there was nothing SHPS would or could do to help (I have years worth of emails to that effect). I'd asked many times if there was anything we could do to increase the volume on my phones - I was told no. We then at some point went to HR and they indicated SHPS could get me a headset that would help. It was perhaps neary a year later I was able to get them.
139
140
141
142
Once we went over to the Arizona switch we lost a sigificant amount of voice clarity on our phones. I'd again asked Cathy if there was anything we could do. I finally went to Voc Rehab for help, and they agreed to finance hearing aids ($6,000) that were designed specifically for use in conjunction with our phones so I would still be able to do my job. These were bluetooth capable hearing aids to be used in conjunction with a bluetooth capable device that was compatible with our phones.
143
144
145
146
I'd asked Cathy if SHPS would help me purchase the device - Cathy said no. I gave all the information on our phones to VocRehab hoping for help, however they gave me a device that is compatible with analog phones, only, although I'd clearly indicated these were VOIP phones.
147
148
149
150
Since I've got somewhat of an electronics background, I'd hoped to be able to convert the device to be compatible with our phones however learned that was not possible.
151
152
153
154
I'd then contacted the manufacturer of our phones and was told there was one device that would in fact be compatible with the streamer (receiving device that communicated with my hearing aids). I'd again asked Cathy if SHPS could possibly help me with that - she said no.
155
156
157
158
I purchased it myself.
159
160
161
162
According to VocRehab, that is not the way it was supposed to go.

I'm not really the one to be answering these questions. I'll get something from Matrix once the review is complete (haven't seen anything yet) but I'm sure you'll get the same information at the same time. I really don't know what other paperwork you'd need to fill out for the disability piece. Mark?

Do we have any word on whether my FMLA application has been reviewed? If so, for what length of time, please? Also, is there anything I need to complete paperwork-wise for the disability? (I'm uncertain of what the distinction is between short-term and long-term: I'd thought mine would be short-term although you'd stated long-term below?).

Sorry . . . I was trying to gather the answer and got various opinions. When I put them all together this is what we get:
303
304
305
Until your long term FMLA is approved, you must call in every day. Once we get the documentation from the absence folks that says: Vicky will be off from [begin date] to [end date], you won't need to call in any longer until after the last approved date.
306
307
308
With intermittent FMLA, you would need to call everyday.
309
310
Hope that answers the question, let me know if it doesn't.
311
312
Thanks!

I'd tried sending the below email via my home email a couple of times however apparently it did not go through. See below text & respond, please? My MD stated I cannot work for 6-8 weeks (that time frame pending Matrix review, of course). Am I still required to notify Workforce now each morning prior to my shift? I'd asked Mark Tatum and he deferred me to you.

Hi Blanca -
530
531
I'm on with my MDO again now, speaking with Violet w. Dr. Wood's office - she indicated they have sent everything you'd requested. She said she will contact you to see what else it is you may be missing as she's sent whatever was requested and she is not aware of any pending requests.
532
533
Thank you so much - anything you can do would be greatly appreciated. Do you think I need to contact an attorney?
534
Best Regards,
535
Vicky

Good Morning, Blanca -
575
576

577
578
What is the status of my disability claim, please? This claim was filed on September 23, and I have not had any income since then. Please - I need help. Do you think I need to contact an attorney?

Hello Blanca -
586

587
Attached please find the MD's notes from my endocrinologist, Dr. Donald D. Wood. When we'd previously spoken you'd indicated this is all that you'd needed in order to process my disability claim.
588

589
Anything you can do to expedite this claim would be greatly appreciated. I have not had any income in a month. I no longer have heat in my home - tomorrow night it is expected to reach the 30's. I was unable to make my mortgage payment last week. My homeowners & car insurance are both overdue. I have no paper products left in my house whatsoever so you can imagine what that is like with this flu or whatever it is.
590

591
I currently have no mobility in my head or neck.
592
Body aches everywhere.
593
Severe fatigue (constant)
594
Dizziness.
595
Loss of appetite.
596
Freezing cold (constant).
597
Insomnia.
598
Constant headache.
599
Severe tinnitus.
600
Nausea.
601
I have had the above for months, and they are getting progressively worse.
602
I do have benign paroxysmal positional vertigo which was diagnosed by Heuser Hearing Institute here in Louisville, however I am unable to continue treatment with it as I simply do not have the co-pay.
603
I do have an adrenal adenoma.
604
I currently have either the flu or bronchitis which I cannot get treatment for because I do not have the co-pay.
605
I had a ruptured brain aneurysm back in 1998 (major subarachnoid hemmorage) which has left me with a few problems, as well as a periventricular shunt.
606
I have a vertebrobasilar insufficiency bought caused by the aneurysm, diagnosed by Dr. Walter Olson, a neurology specialist in movement disorders.
607

608
Just to mention a few. Dr. Wood now suspects fibromyalgia. I do not have the copay to see a specialist.
609

610
Please, if there is anything you can do to complete my claim I would greatly appreciate it.
611

612
Thank you,
