  16. You are receiving this e-mail communication because our records reflect that you have requested, or are currently on, an intermittent FMLA Leave of Absence.

  18. As previously communicated to all SHPS associates, effective January 01, 2011 SHPS began utilizing Matrix Absence Management for our FMLA administration needs.

  20. As such, going forward the next time you report an absence due to an intermittent FMLA leave, you will need to utilize the attached Time Tracking Sheet to report your absence to Matrix. A Leave Coordinator from Matrix will then contact you to review your existing Health Care Provider Certification and will ask you to provide updated information regarding your leave of absence. If you no longer need intermittent leave, please notify Matrix at the number provided below.

  22. If you still need intermittent leave, each pay period you will still be required to submit a timesheet indicating the time you have taken toward your intermittent leave for that pay period. The timesheet is to be completed, signed by your supervisor and faxed back to Matrix. Matrix will then notify SHPS of the time you have requested.

  44. I did see my MD this afternoon, and he stated I cannot return to work for 6-8 weeks. He said he cannot have the FMLA certification filled out today - that they hope to fax it to SHPS by Friday (to (502) 297-5345 which is the fax number on the form I have - please let me know if this number has changed) if possible, although he also stated per Federal Law they have 30 days to complete this form. He also stated per the FMLA form from SHPS I have 15 calendar days from the time I'm given the form from HR. I did have a copy of the blank form here at home & used this one today (although Mark Tatum did offer to email it to me). The right foot is fractured and the left ankle is sprained.

I also spoke with Mark Tatum a little while ago and asked if I am required to call in each day until the FMLA was approved/denied by Matrix, and if I understood correctly I was to direct this question to you. Is this necessary? I do not wish, after all, to create more work or incovenience than necessary for Marshall or Workforce.

I have made silly errors in the past (like leaving when I could neither hear or breathe) that nearly cost me my job - I am simply trying to avoid that now and attempting to ensure I have covered all my bases on this end since my past experiences in working with my immediate supervisors have not been successful. So forgive me if I tend to stay on top of things to ensure everything is done correctly.

  139. Just as with the problems I'd had with difficulty in hearing my calls at work. Cathy repeatedly told me there was nothing SHPS would or could do to help (I have years worth of emails to that effect). I'd asked many times if there was anything we could do to increase the volume on my phones - I was told no. We then at some point went to HR and they indicated SHPS could get me a headset that would help. It was perhaps neary a year later I was able to get them.

  143. Once we went over to the Arizona switch we lost a sigificant amount of voice clarity on our phones. I'd again asked Cathy if there was anything we could do. I finally went to Voc Rehab for help, and they agreed to finance hearing aids ($6,000) that were designed specifically for use in conjunction with our phones so I would still be able to do my job. These were bluetooth capable hearing aids to be used in conjunction with a bluetooth capable device that was compatible with our phones.

  147. I'd asked Cathy if SHPS would help me purchase the device - Cathy said no. I gave all the information on our phones to VocRehab hoping for help, however they gave me a device that is compatible with analog phones, only, although I'd clearly indicated these were VOIP phones.

  151. Since I've got somewhat of an electronics background, I'd hoped to be able to convert the device to be compatible with our phones however learned that was not possible.

  155. I'd then contacted the manufacturer of our phones and was told there was one device that would in fact be compatible with the streamer (receiving device that communicated with my hearing aids). I'd again asked Cathy if SHPS could possibly help me with that - she said no.

  306. Until your long term FMLA is approved, you must call in every day. Once we get the documentation from the absence folks that says: Vicky will be off from [begin date] to [end date], you won't need to call in any longer until after the last approved date.

  588. Attached please find the MD's notes from my endocrinologist, Dr. Donald D. Wood. When we'd previously spoken you'd indicated this is all that you'd needed in order to process my disability claim.

  590. Anything you can do to expedite this claim would be greatly appreciated. I have not had any income in a month. I no longer have heat in my home - tomorrow night it is expected to reach the 30's. I was unable to make my mortgage payment last week. My homeowners & car insurance are both overdue. I have no paper products left in my house whatsoever so you can imagine what that is like with this flu or whatever it is.

  594. Severe fatigue (constant)

  596. Loss of appetite.
  597. Freezing cold (constant).

  602. I have had the above for months, and they are getting progressively worse.
  603. I do have benign paroxysmal positional vertigo which was diagnosed by Heuser Hearing Institute here in Louisville, however I am unable to continue treatment with it as I simply do not have the co-pay.
  604. I do have an adrenal adenoma.
  605. I currently have either the flu or bronchitis which I cannot get treatment for because I do not have the co-pay.
  606. I had a ruptured brain aneurysm back in 1998 (major subarachnoid hemmorage) which has left me with a few problems, as well as a periventricular shunt.
  607. I have a vertebrobasilar insufficiency bought caused by the aneurysm, diagnosed by Dr. Walter Olson, a neurology specialist in movement disorders.

  609. Just to mention a few. Dr. Wood now suspects fibromyalgia. I do not have the copay to see a specialist.

  611. Please, if there is anything you can do to complete my claim I would greatly appreciate it.
