SPECIAL CASE 1: TERRA-XEHANORT (translation by slm)
Terra-Xehanort continued to exist by taking Terra-Xehanort's heart from the past, and placing it into Terra’s heartless body. Originally, Terra-Xehanort had been in a constant state of Terra's heart and Xehanort's heart fighting for leadership, with Terra's heart bouncing back and forth between his body and the monster that appeared behind Terra-Xehanort. When Ansem and Xemnas were slain, Terra's body was recompleted alongside Xehanort and Terra's heart was placed back inside the monster while Xehanort made his body a vessel again.

SPECIAL CASE 2: XION
At the time of KH3D, the real Organization XIII was absent one member. There, Xion filled the role. She exists via the transfer of the heart of the replica doll Xion from the past; she has almost no memory of Roxas and Axel. However, a call from Roxas within Sora triggered Xion's heart, which was in Sora's heart. It was absorbed by her body, which had the ability to absorb Roxas and Sora's power, and Xion's memories and existence were restored. Thus, memories of her that were lost in Axel and others were recovered.
