Enterprise Survey

Sometimes a coupon requires a Membership # from an affiliate. Enterprise uses 'Member Number' as the caption for the field. This makes it seem like the field is asking for the Enterprise Plus member number. Please add the affiliate name to the start of the caption. This is harder that displaying a static string. But you already retrieve the affiliate name to display coupon info. Concatenating the affiliate name to the static string is not hard.
