Self Employment failed me ODSP failing me currently

I Started My business June 8th 2022 I wasnt on ODSP till November. I was Neglected no one followed up in regards to the self-employment pathway so im basically operating this business illegitimately due to missing paperwork from not being taken seriously enough resulting in a failure to address so many questions as to how further officiate my business

how do the business statements that were submitted play into my business filing I need more information and i feel like it may look as if i was informed but after applying for my business I never received any follow up or contact in regards to it so im confused as to why the delay has lasted this long, Ive started having fears of fraud due to not having so many of my concerns addressed. Which has also caused me to become discouraged and disinterested in working. While also being unable to apply for parttime work possibly due to a police investigation informing employers ive never experienced such difficulty obtaining employment with my qualifications. So im just confused. Looking for clarity

I have also contacted my primary health care practitioner in regard to correcting a diagnosis that was incorrectly filed. I would like some information on how updating my diagnosis will effect my assistance, will it require an internal review or revision / update of my file ?
