Pictures and signatures: Upload the photo to IMGUR.COM and paste the link onto the order form. Take a photo against a white wall. Make sure there is good even lighting across your face and also make sure there are no outside light sources that would create a shadow across your face or neck. For signatures, use black sharpie to sign a full white sheet of paper. Do not use pen.

Use correct formatting for dates and weights. It is 05/06/1994 not 5/6/94.
