  1. Having a forked road, where one way is to the save point, and the other to the boss. But then taking the path to the boss and getting wrecked from inexperience.

  3. Going through a straight path, fighting some enemies and end up going backwards because the camera turned during battle and confused where you were going.
  4.  
  5. Making you go through a lot of trouble, and starting you far back when you lose.
  6.  
  7. Being lost in a labyrinth, and always accidentally going the right way, missing all the treasure.
  8.  
  9. Levels where you have to climb high up, and if you fall down, you'll have to start all over again.
  10.  
  11. Levels with slippery ice floors with death traps everywhere (Crash Bandicoot)
  12.  
  13. Levels that become dark, where you can't see anything until the area flashes from lightning for a split second.
  14.  
  15. When you can't skip the long credits, and the names are scrolling by too fast to read anyways.
  16.  
  17. When you watch an intro cutscene before the title screen, just to watch the same cutscene when you start the game and can't skip it.
  18.  
  19. When a game doesn't teach you how to crouch, but you need it to get through a part of the game. Making you lost for hours until you look it up.
  20.  
  21. Having a missable weapon/item that you need for survival, hidden well so that you'll need to restart the game and use a guide to find it.
  22.  
  23. Missables that you can come back for are a pain, especially when they tease you right when you exit an area and can't come back until you restart the level.
  24.  
  25. Collectibles where you have to kill yourself in order to get it, no other way.
  26.  
  27. Having a hard puzzle that will force you to use a guide, or sit there for an hour trying to figure it out yourself.
  28.  
  29. Being teased by an almost completed puzzle where you only need one more piece to complete it, but you can't get the last piece in without moving the others.
  30.  
  31. Having a hard boss where you'd lose a lot, making you watch an unskippable cut scene every time you want to retry.
  32.  
  33. Powerups or items that disappear, right when you're about to pick it up
  34.  
  35. When you're forced to fight a tough enemy in a tight hall, where you can barely avoid it's attacks.
  36.  
  37. When you pay to stay at an Inn, then the game's plot forces you to sleep anyways
  38.  
  39. When you have to talk to one specific NPC to advance the plot, being lost the entire time making you look up a guide
  40.  
  41. Enemies kicking my ass, making me use a full healing item, just to find a device that fully heals me for free anyways.
  42.  
  43. Enemies that use the same, obnoxious move that knocks you over every time.
  44.  
  45. Enemies that shoot at you from far while you're trying to fight off other enemies.
  46.  
  47. Enemies that fly above you and annoy you, while you have almost no way of hitting them from the lack of ranged weapons.
  48.  
  49. Scientist bosses that runaway, making you fight their creation. Then later when it's time to fight them, they hide in a force field and use smaller enemies to hold you back
  50.  
  51. Bosses that use moves you can't dodge or block, forcing you to take a hit.
  52.  
  53. Bosses that can heal themselves, or regenerate body parts. Forcing you to be quick and efficient.
  54.  
  55. Confronting an epic boss fight with epic music, where you feel like a hero, but then losing because you need to learn the bosses moves first. Not able to win on your first try and making the character look stupid for losing.
