  5. We had indicated to you previous that ultimately it is your responsibility to provide any information you wish to have considered during the course of the appeal. Previously, we had tolled the timeframes for making a determination, in order to obtain your medical records. Please remember, we are trying to determine if you remain disabled beyond February 28, 2013. We believe that we have obtained the relevant medical records addressing that time period. As you know we have requested an IME and the IME has been scheduled. The timeframes are tolled while we await your IME, and the receipt of a copy of the IME report.

As individuals applying for the continuation of their LTD benefits frequently continue to treat with their providers, there has to be a cut off time in which we stop requesting medical records during the appeal process. However, that said, we will continue to have any additional medical evidence that you submit, reviewed in conjunction with your appeal. Therefore, as previously communicated to you, please feel free to send us anything additional you would like to be reviewed in conjunction with your appeal.

As previously explained, as an IME has already been requested, you are welcomed to send in any additional medical records you would like the IME physician to review. As such, we will not be requesting any additional records, but we will certainly forward anything additional that you obtain and submit to us, to the IME physician.

  51. You are very welcome. I did have the inguinal hernia repair surgery. The surgeon was Dr. Kevin O'Koon, MD, Baptist Surgical Associates, Kresge Way, St. Matthews, KY - If you could please get the records I would appreciate it. FWIW they did not ask for any type of an ID prior to my surgery that day and I thought that was a very quick check in. Kind of like the express TSA thing. I liked it alot. The anesthesiologist didn't even want to hear about any heart problems or electrolyte imbalances either so it was a quick interview there as well. But O'Koon was fabulous.

I'd also tried to see if I could get the 2 fibroids done at the same time as I'd understood from Sven Jonssons office they were on the ovaries and they were 2 large fibroids and was hoping to avoid being opened up twice. I did ask for them to fax me the reports but their entire telephone system is not compatible with Time Warner's so I put in a trouble ticket for them yesterday (Baptist East - Time Warner Trouble Ticket # 18477002). Dr. O'Koon said he didn't mess with ovaries so I made an appt. with an OBGYN named Coatheimer. I messed up w. the appt. with Coatheimer and it was the day after the O'Koon surgery so I tried calling them the day before to see if that was going to be a problem. I spoke with Coatheimer's office three times Wednesday - I wasn't .if they would have a problem doing the exam so soon after the surgery. They said the MD was going to be in appts. all day and finally at the end of the day they asked me if I was able to "take a speculum" and I said I was sure I was so they said to come on in.

Needless to say she (Coatheimer) said I should not have come the day after the surgery and all she had to do was look at the echo report to see the fibroids were so small they did not need to be operated on, that they were on the upper portion of the uterus and that they were benign, and that my uterus was dried up. I did mention to her that she might want to fix the 5" gap in their venetian blinds next to their toilet where they'd asked for a urine sample that gave a clear view out into their parking lot and to also find the missing "g" that was obviously omitted throughout all of their paperwork for whatever reasons. That whole place had to be a joke, right?
