  3. - I'd be remiss to not mention the successor to Real Cone Head is out now (https://steamcommunity.com/sharedfiles/filedetails/?id=1385512368), a community contribution fix mod that corrects many small issues.   5. - "Something Wicked This Way Comes" does not lock its respective bosses in Boss Rush (AB+ bosses also oddly lacking from Boss Rush), normal rooms, and Greed Mode, meaning they can still appear despite locked.

  7. > Angel bosses are also not locked behind their achievement, meaning they can still spawn in Boss Rush/Sacrifice Room/other places.

(He was also carried over into the Greed Mode boss pool as an extra note, if fixed)

  15. - Permanently charmed Crown champions and Pulsing Red champions causes opposing enemies in a room to become champions and continually heal respectively, resulting in an unwanted determent.

  17. > While on the topic of Crown champions, they only seem to turn enemies into Yellow (Battery) champions, and Yellow champions have an ID of 1. Is this intentional or was the Crown champion supposed to randomize each enemy?

  19. - Key Bum cannot pick up charged and golden keys, while Bum Friend/Bumbo can pick up lucky pennies (though with no benefit). Intentional?

  21. - Angel bosses spawned via Sacrifice Rooms do not close the doors unlike enemies spawned via buttons, nor do they play their boss fight music.

  23. - Pay to Win does not add a restock box to Hush item rooms.

  25. - When Buddy in a Box rolls certain item effects, such as Proptosis or Tiny Planet, they are also not given any appropriate stat boosts alongside those effects, which can result in poor range/damage.

  27. - With Ghost Pepper + Butt Penny, the Butt Penny fart does not shoot a fire unlike most other sources of farts.

  29. - The item "Purity" is present only in Treasure rooms, but in Greed Mode the item is in Treasure rooms *and* Angel rooms. This seems to imply that Purity is an angel-themed item, and could potentially be added to the Angel pool in normal mode.

  57. - Mom's Box does not double the effects of: AAA Battery, Fish Tail, Monkey Paw, Rotten Penny, Eve's Bird Foot, Cracked Crown, and all 5 Locusts.

  61. - Stoneys are oddly immune to the slow status effect, particularly from room-wide sources (Such as Stopwatch)

  63. - The challenges "I RULE!", "Backasswards", and "Ultra Hard" are all simply unlocked by beating ??? or The Lamb for the first time, despite having a goal to Mega Satan. This can easily spoil the secret of Mega Satan to players who have never beaten or seen him before, especially Backasswards which literally starts at him.

  65. - Champion Brownie still spawns Dank Squirts on death instead of normal Squirts, since the boss was nerfed to spawn normal Chargers instead of Drowned Chargers.

  67. - Little Horn's (item) contact damage overrides all other contact damage items, such is My Little Unicorn or others. This is troublesome since the horn has the lowest contact damage in the game.

  69. - Despite being poop-themed, Skatole, Butt Bombs, No. 2, Brown Nugget, and Hallowed Ground do not count towards the "Oh Crap" transformation. Its been a neglected and nigh-unobtainable transformation for so long.

  73. > Spider Baby also has plenty of spider-themed items that don't count towards it. Potential list: Daddy Longlegs, Sissy Longlegs, Bursting Sack, Juicy Sack, Sticky Bombs, Infestation 2, and Parasitoid (Debatable, though Parasitoid would ideally count towards both Spider Baby and Beelzebub).

  83. - Holy Light tears don't emit light like they could/should, unlike something like Technology which emits light.

  93. - Delirious and Eden's Soul items (12-charge active items) are not consistent with Mega Blast in regards to batteries, as Mega Blast isn't immediately filled by batteries while Delirious and Eden's Soul are.

  97. - Callus does not prevent damage from stopping the Greed Mode button, despite being a spike

  99. - Back when Afterbirth added new challenges to the menu, it removed the unique doodles and graphics that decorated the Rebirth challenges, and most notably the strikethrough effect. The "life gives you lemons" strikethrough used to go down the side and cover the whole title. Now instead, it has a generic strikethrough effect and fails to cross out the whole title. There's a simple mod (http://steamcommunity.com/sharedfiles/filedetails/?id=1255365718) that restores the graphics missing since Rebirth, while keeping the AB and AB+ challenge graphics untouched.

  101. - Going to the Blue Womb with the "N0RE TVRN" (Point of no return) seed will softlock the run, as the Hush boss door won't spawn.

  103. - The seed "G0NE S00N" (Pickups time out) causes the end chest dropped by final bosses to "time out," softlocking the run and making it uncompleteable.

  105. - The mod toggle menu paper would benefit from being wider, as currently any mod that doesn't have a short name will have its name extend off the paper.

  107. - The Thing continually fires its tentacle even when it doesn't shoot creep, making the enemy have a hard tell to them. That may be slightly intentional as the tentacle is also a hitbox, but alternatively The Thing could keep its tentacle out for a much longer time when it shoots, and only retract it when it's ready to fire creep again.

  109. - Clicker can transform you into the character you currently are. Which is pointless.

  119. - Lua: If a player gets hurt from walking into a fireplace, it has the wrong damageflag value of 0, instead of 2 for firedamage.

  121. - Lua: Removing Bone Hearts with AddBoneHearts(-1) doesn't always update the HUD properly, and the act of doing so is rather buggy overall.

  123. - Lua: "MC_EXECUTE_CMD callbacks tend to crash if you return anything. I think there's a memory allocation issue here that's causing the console to print garbage strings or segfault." - /u/warmCabin

  125. - Lua: "The Steam Web API for the game is broken; `GetUserStatsForGame` is supposed to return the list of earned achievements for a given (public) Steam user but currently only returns an HTTP 500 Internal Server Error."

  127. - Considering that the super-rare "Bob" transformation includes exclusively bomb items, the transformation could also make you immune to explosions, just like Sloth, so it actually has some usefulness when you obtain it once in a blue moon.

  129. - Currently, there are no in-game hints that doing things like beating a run without pickups besides keys, beating The Lamb in under 20 minutes, 5-winstreak as 5 different characters, daily runs, etc. will unlock achievements. Potentially, the Rules Card could get a bit more use if it had some new messages to hint at various hidden secrets, as the card has been nigh-useless since Rebirth.

  131. - There are 3 special unlockable co-op babies that have no (documented) effect: Cute Baby, Cracked Baby, and Picky Baby, which makes their special status odd. Cracked Baby for example looks like it was designed to imitate the effect of Cracked Dice, but it does no such thing.

  133. - The Mind lacks a map icon unlike all other mapping items

  135. - Mega Satan: A few boss spawns default to spawning in the bottom left instead of the bottom center (Super Envy, Fallen Uriel/Gabriel), making the spawning look uneven.

  137. - Mom's Dresser cannot spawn in Greed mode. This mod's suggested implementation (https://steamcommunity.com/sharedfiles/filedetails/?id=1354407919) is that they could have a chance to spawn in the Silver Treasure Room.

  139. - The "Victory Lap" prompt shouldn't appear until the player beats The Lamb their second or further time (Could be tied to The Gate is Open). For new players, this prompt comes out of nowhere and could have them miss seeing the ending they just unlocked.

  141. - Victory Lap related unlocks reportedly (At least with "Once More With Feeling") fail to unlock if you have mods enabled, even when mods are "unlocked" from beating Mom's Heart.

  143. - Victory Laps count as a loss to the winstreak when dying during them, even though you've technically already won the run.

  147. - Dagaz has no interaction with Cursed Eye despite being a Curse purifier, unlike Black Candle.

  149. - Here's an oddity/bug I've noticed about my own personal item in the game: Mystery Gift. Whenever Mystery Gift is used, it uses a black smoke effect when spawning the pedestal. This smoke effect was actually intended to be white, not black, most of the time, and only be black if it spawns Lump of Coal. Similarly, I intended the smoke cloud to be a brown fart cloud when The Poop is spawned. The sound effects are correct, but not the visual effects.
