  21. [3] - 07/2460 - Surgical procedures carried out to repair extensive burn damage to subject's legs following chemical-related accident, third degree burn repairs performed successfully with significant scarring expected.

  28. [1] - Prescribed 07/2460 - Oxycodone, 2u. Taken several times daily following chemical burns to legs, abdomen and lower back, or as necessary, not exceeding six doses per day. - Still valid as of 09/2460.
  29. [2] - Prescribed 09/2460 - Tramadol, 1u. Taken several times daily in conjunction with 2u Oxycodone following diagnosis of chronic neurological pain, or as necessary, not exceeding six doses per day.
  30. [3] - Prescribed 12/2460 - Fluvoxamine, 1u. Taken three times daily to treat muscle weakness following surgical application of regenerative membrane to nervous tissue in legs.
  31. [4] - Prescribed 12/2460 - Olanzapine, 1u. Taken three times daily along with 1u Fluvoxamine to treat muscle spasms in legs associated with regenerative membrane application.
