  144. I just got a packet of Financial Request information I opened today from kentucky One Health - Patient Account #D1408301960 to help with Financial Assistance. They want it back within 14 days?

  146. I cannot even remember the last time I was in Jewish Hospital (Eastpoint) as it's been closed for quite sometime - and they want this back in 14 days? I haven't been there in years. I'd asked for a Healthcare Advocate as I have brain damage due to a massive subarachnoid hemohage. I cannot even remember to pay my electric bills here in my home so I do not know how they expect me to complete 6 pages of paperwork? All they did was run unnecessary tests and additional radiation to fry my brain even more. It would be a miracle if I do not have a brain tumor, leukemia or lymphoma at this juncture after dozens of CT scans and x-rays given.

  149. HEALTHCARE ADVOCATE - been asking for one since roughly 2008 from every medical facility in
  150. Louisville.

  152. At this juncture I have given up and cut off all Louisville medical facilities. Now it's just a matter of which one of you are going to get a piece of my house when I die and the way things are going it won't be in the too distant future. I'm about ready to sign over my medical Case Mangement and power of attorney to Chase bank because at least they will sit down with me and advise me when to pay my mortage, so you'll probably have to deal with them at some point or write off however much you claim I owe you. Perhaps in the future you'll keep your unnecessary medical procedures in check (like limiting CT Scans to less than 12 per year as you did to me and so many others I've spoken to).
