- 1. Defeat Mom to unlock the Womb.
- 2. Defeat Mom's Heart 11 times to unlock It Lives.
- 3. Defeat It Lives to unlock Sheol and Cathedral.
- 4. Defeat Satan five times to unlock the Negative, and defeat Isaac five times to unlock the Polaroid.
- 5. Defeat Satan after taking the Negative to proceed to the Dark Room, and defeat Isaac after taking the Polaroid to proceed to the Chest.
- 6. Defeat The Lamb in the Dark Room, and defeat ??? in the Chest.
- 7. Bomb the statue in an angel room and kill the summoned angel boss to get a key piece. Do it twice in the same run to complete the key and open the giant door in the Dark Room or the Chest to fight Mega Satan.
- Defeating Mom in less than 20 minutes opens a door to the Boss Rush chamber.
- Defeating It Lives in less than 30 minutes opens a door to the Blue Womb, where you fight Hush. Defeating Hush unlocks the portal to the Void, where you fight Delirium. The Void is always accessible by defeating Hush and now has a chance of appearing after defeating Mom, It Lives, Isaac, Satan, ???, The Lamb, or Mega Satan.
- Defeating each of It Lives, Isaac, the Boss Rush, Satan, ???, The Lamb, Mega Satan, Ultra Greed, Hush, and Delirium for the first time with a particular character gives that character a completion mark for the defeated boss. Doing the same on Hard mode (or Greedier mode) adds a red outline to the completion mark.
- A new item is unlocked for completing each character/boss combination, as well as for beating It Lives on Hard mode and for getting all completion marks on Hard mode with each character. The items unlocked by defeating Mega Satan, defeating It Lives on Hard mode, and for getting all completion marks on Hard mode are all multiplayer babies and thus aren't a priority, with two exceptions:
- • Defeating It Lives on Hard Mode with Apollyon unlocks Smelter
- • Collecting all completion marks on Hard mode with The Lost unlocks Godhead
