Penny Plan:
Premium: $125 for Individual Plans, $250+ for Family Plan(Depending on number of kids - $60 per kid for up to five kids, and then $125 per kid after that)
Copayment: $25 Generic Drugs, $35 Routine, $50 Specialist
Monthly Deductible: $900

Bronze Plan:
Premium: $200 for Individual, $400+ for Family Plan($90 per kid for up to five kids, and then $180 per kid after that)
Copayment: $20 Generic Drugs, $30 Routine, $45 Specialist
Deductible: $700


Silver Plan:
Premium: $300 for Individual, $600+ for Family Plan($100 per kid for up to five kids, and then $250 per kid after that)
Copayment: $15 Generic Drugs, $25 Routine, $40 Specialist
Deductible: $500

Gold Plan:
Premium: $400 for Individual, $750+ for Family Plans($110 per kid for up to 5, $300 after that)
Copayment: $10 Generic Drugs, $20 Routine, $35 Specialist
Deductible: $300

Platinum Plan:
Premium: $500 for Individual, $900+ for Family Plans($120 per kid for up to 5, $350 after that)
Copayment: $5 Generic Drugs, $10 Routine, $20 Specialist

Employers will subsidize between 10-25% of additional premiums. Agents will have between 300 and 330 people that they cover.
