Notes:

Medical History:
 - 23/06/2461 - Operation to repair damage to lungs and scales from phoron fire. Successful, scarring remains the only complication.

Psychiatric Notes:
 - Patient formerly displayed a reticence for sharing information, which seems to have been reduced after experiencing a traumatic event.
 - Patient notes a history of substance abuse issues and difficulty remaining off these substances. She claims that at the moment, the only intoxicant she uses is ethanol (patient clarified that she takes it with dylovene; possible risk-taking behavior). She also stated that she knew of its interaction with her medication and did not report any side effects, which implies she has not been taking her medication.
 - Patient notes guilt over a perceived burden, which is to be expected with her depression of major depressive disorder. She denies any suicidal thoughts or actions.
 - Patient reports a past failed marriage (arranged), and states a distaste for all men, to the extent that she claims to lack any attraction at all to them. Despite this, she denies being asexual, though reports no significant attraction to anyone. Possible traumatic complications from her arranged marriage, though the reporting doctor believes this is unlikely given that she lacks any other signs.
 - Patient reports difficulty focusing on a singular task and an inability to complete personal projects in a timely manner, except for some times when she reports large amounts of activity on one particular topic; she reports that this mainly involves chemistry, her chosen field.
