2) Where are you most active on the Hypixel Network? 
Typically, you will find me in game more than on the forums. When I'm not playing games, I normally hang out in lobbies, such as Skywars lobby 1. If you couldn't have guessed, my main game I play on Hypixel is Skywars. I do play much more though, most games are fun and I have begun to quest. The forums I post occasionally, but I browse them 6 days out of 7. I definitely prefer being in game when talking to others.
3) How much time do you have to contribute to the role?
I have no other servers I play currently, so my full time would be dedicated to Hypixel. I spend about five hours a day during school days when nothing is preventing me from doing so, and around eight hours on weekends.
4) Have you ever been banned or punished on any server? If so, please include details.
Oh boy, here we go... Keep in mind on none of these servers I have cheated.
Yes, I have been banned on quite a few servers. INCLUDING Hypixel, sadly. Let me lists the reasons here.
Hypixel: I believe I've been banned four times on the Hypixel network. The first ban I remember clearly, and am not proud of it. I think it occurred in 2015, I was semi-new to the server, and I told a player to "kys".
I was banned for a week, and after that happened I never used a death threat again, in fear I would be banned from the server again. My other three bans were for "Kill-aura". Every single ban I've had on hypixel is completely false, all were videos recorded in third person, which makes it quite unclear to see exactly how I was "cheating". Before moving to Hypixel, I did use a hacked client on other servers. Again, I am not proud of it but it is in my past, and I have learned from it.
OP-Factions: I was banned once here for Kill-aura. The guy recording was incredibly laggy, and the video is deleted along with his channel.
Desteria: This was my first server I had played on (that I remember, around late 2014), the one that taught me to PvP and become better. I was devastated to see myself banned one day, the first server I was banned on. I honestly thought I would never get to play on it again, but my friends told me I could appeal. The staff overruled the ban and I resumed playing.
Fadecloud: I only played kitpvp on this server, and became semi-known and friends with a few. It has a leveling system from what I remember and I was high in it was well. A staff member decided to ban me because I looked weird when PvPing, although I never hacked on this server. I am unbanned now as I believe the server cleared its bans quite some time ago.
[NOTE] I did start cheating on small servers and have gotten banned on them (badlion only big server), but I will be sure that never happens again. I have deleted my hacked client and shall wait until I feel I have learned my lesson to submit this application.
5) SCENARIO: Suppose it's your first week of being a Helper and you mute a well-known player. Their guild mates start spamming your twitter calling you a bad Helper and telling you to unmute them. What would you do?
I would respond as collectively as possible, telling them the rule he has broken. If they resume spamming on twitter, I will just ignore them.
6) Tell us about one of your best moments on the Hypixel server
When Skywars was released I immediately began playing it. Within a few days I was not aware of how much time I had spent on it, and noticed I was on the leaderboard. It was the first time I had been on it for anything, and was so happy and managed to get this screenshot. (MLG Alex skin with no texture pack haha)[IMG] 
Back in the days where ffs ruled. He was crazy good. Anyways, mostly everyone you see here has quit the server, and I was one of the few who continued playing Skywars. I had a goal, to be number one, and for many weeks Skywars was the only game I played.
7) Tell us about a time you made a mistake within the last year? How did you deal with it? What did you learn?
In the past I have been muted quite a bit for making on the edge jokes on Hypixel. After getting five days worth of a mute, and appealing with it being denied, I figured it's just as bad as getting banned, if you can't talk to your friends on the server. Without them it's not much fun. I've now learned that these controversial jokes are OK to think, not to say.
8) If you have any, what hobbies or activities are you involved in outside of Minecraft ?
I am on my schools Varsity soccer team, this is one of the activities that will slightly lower my time on the server. I am also on a club soccer team.
9) Do you have any previous server moderation or leadership experience?
I do not, but I will explain the closest I have gotten to a staff position. I was well respected and known on a server I have listed above in question 4, OP-Factions. I had created a staff application that everyone approved and gave +1's, not a single person gave a -1 on that thread. I helped out in game to those who needed it and answered questions people had on the forums. However, I was too late in the application for staff on that server, for the owner had decided his time was up, and decided to leave. My thread was left to stay for the months to come when eventually, the website was reworked, and everything past created gone.

10) SCENARIO: "You come home from school/work and have ton of homework/work to do, you think to yourself I'll just take a 5 minute break before I start and get on Hypixel. The next thing you know 2 hours have passed and you've been on the server all that time." Does this scenario sound familiar to you? 
For me, I normally finish most of my homework in school, since I have an Academic Prep. If this is the occasion, I would make sure to finish my homework before getting on Hypixel, so I could dedicate 100% attention towards it. I wouldn't allow this situation to occur.
11) SCENARIO: You're in a busy lobby as a helper, questions are flying at you rapidly, you can just about keep up. You're getting messaged by 2 different players both asking you to get them a moderator for a hacker, someone then pokes you on teamspeak telling you that there is someone blasting music in their channel. What do you do? 
I would make sure I know each offense first of all. Whoever has messaged me first I would answer first. Let's say the two people asking me for a moderator asked first, I would pass it on, and then answer them so. I would then tell the lobby I am attending another problem in teamspeak and will be back in a few. I would then address the issue with the person blasting music and punish accordingly.

None so far.

I am ready to dedicate what is needed to provide for this server. I have countless hours on it, more than any other server combined I have played on. Re-reading this, it's making me giddy ahaha. Knowing I can be someone for once, for a server I have favored above all. Thank you for reading.
