132.

133.
If the CCW license is desired for self-protection, the protection of others, or for the protection of large sums of money or valuable property, you are required to explain and provide good cause for issuance of the license. For example, has your life or property been threatened or jeopardized? Explain incidents and include dates, times, locations, and names of police agencies to which these incidents were reported.
134.
135. Details of Reason for Applicant desiring a CCW License:
136.
137. The reason I applied for the CCW License is due to the fact that as a trucker, you transport a lot of cargo, which is worth thousands of dollars. And most of the highways are deserted, as well as the industries and the factories. This makes it a favourable environment for thieves to linger around.
138. I have been a victim of a robbery once before, where my truck carrying five thousand dollars worth of cargo was stolen, leaving me stranded in the middle of no-where.
139.
140. Around 3 months ago, at around 14:00.
141. As per my daily routine, I arrived at the SA Food Processing Plant to load a few crates of Meal and deliver them to the Restaurants and Fast Food Joints that required them. The plant was deserted, and most of the workers had left for their lunch break. As soon as I finished loading my truck with the cargo, a man, with his face covered, ran up to me with a gun pointed to my face. He ordered me to hand over the truck keys and stole my wallet, as well as my mobile phone. Since I complied with him, he left me unharmed, but left with my truck which was carrying a lot of Cargo. At that point, I was stranded and had no way to notify the police agency. I managed to get a lift from a fellow trucker, who I explained the whole incident to. We then proceeded to inform the Sheriff's Department about the incident and a complaint was lodged.
142.
143. 25/JUL/2016, at around 17:30.
144. I left from the Hucklefield Logistics Headquarters and was on my way to Shafted Appliances in Las Venturas. When I was on my way to the LS-LV Highway, I noticed a white admiral follow me, mimicking all the turns I took. I noticed something was not right and I started to slow down just before getting onto Central Exchange. That's when the admiral took the ramp which enters the North South lane of the highway. I then got onto the highway and was headed to Las Venturas. Just before I reached the tolls, I noticed the same white admiral following me, at high speeds. I got past the toll and decided to pull over. The White Admiral slowly drove past me as both the occupants eyed me, one in the front, one in the back. They drove ahead and took a U-turn and stopped. At this point, I yelled out to them asking what they wanted, and received no reply. They then took another U-turn, driving past me once again and then disappeared into Las Venturas. I remained where I was and dialed 911, and explained the whole incident. In about 3 minutes, and couple of police cruisers arrived at the Toll Booth and were questioning me, when they got radio'd by a colleague telling them that they had spotted a white admiral, that was parked in an alley at the SA Food Processing Plant. That re-assured me about their motives, that they wanted to rob a few unsuspecting truckers. I was taken to the plant and was made to identify the vehicle. The deputies continued to pull them over and follow routine.

146. 27/JUL/2016, 17:30.
147. I was at my apartment complex and was taking pictures of the complex as well as my apartment as i was posting an advertisement about it. As soon as I stepped out my apartment, I noticed an armed suspect inside the complex, hurrying about as I could hear sirens blaring outside. At this moment, I really didn't know what to do. Luckily the armed suspect did not notice me and quickly broke into one of the apartments, taking refuge. I proceeded to calmly step outside the complex with my hands in the air, and let the officers ID and frisk me, confirming that I wasn't a suspect. In case I had been noticed by the suspect, and opened fire upon, I would have no way to defend myself. He would have broken into my apartment as well, to take refuge and would have held me hostage as well.

155.
The licensee is responsible for all liability for, injury to, or death of any person, or damage to any property which may result through any act or omission of either the licensee or the agency that issued the license. In the event any claim, suit, or action is brought against the agency that issued the license, its chief officer or any of its employees, by reason of, or in connection with any such act or omission, the licensee shall defend, indemnify, and hold harmless the agency that issued the license, its chief officer or any of its employees from such claim, suit, or action.
156.
157. The licensee authorizes the licensing agency to investigate, as they deem necessary, the licensee’s record and character to ascertain any and all information which may concern his/her qualifications and justification to be issued a license to carry a concealed weapon and release said agency of any and all liability arising out of such investigation.
158.
159. While exercising the privileges granted to the licensee under the terms of this license, the licensee shall not, when carrying a concealed weapon:
160.
161.
• Violate any of the license usage or prohibition criteria guidelines

163. • Be in a place having a primary purpose of dispensing alcoholic beverages for on-site consumption.
164. • Be under the influence of any medication or drug, whether prescribed or not.
165. • Refuse to show the license or surrender the concealed weapon to any peace officer upon demand.
166. • Impede any peace officer in the performance of his/her duties.

168. • Unjustifiably display a concealed weapon.
169. • Carry a concealed weapon not listed on the permit.
170. • Carry a concealed weapon at times or circumstances other than those specified in the permit.
171.
172. Pursuant to U.S. Government Code – Title 49, Chapter 26, Section 1472(1) and Federal Aviation Regulation 121.583, a license to carry a concealed weapon does not authorize a person to carry a firearm or any dangerous weapon aboard commercial airlines. Further, a person must declare that he/she is carrying such firearm or dangerous weapon BEFORE entering the boarding area of an air terminal where the security checks are made. Such violation can result in arrest by law enforcement.
173.
174. Any violation of these restrictions or conditions may invalidate the CCW license and may void any further use of the license until reinstated by the licensing authority. Any arrest for a felony or serious misdemeanor, including driving under the influence of alcohol and/or drugs, is cause for invalidating the license.
175.
176. -
177.
178. I hereby accept and assume all responsibility and liability for, injury to, or death of any person, or damage to any property which may result through any act or omission of either the licensee or the agency that issued the license. In the event any claim, suit or action is brought against the agency that issued the license, its chief officer or any of its employees, by reason of, or in connection with any such act or omission, the licensee shall defend, indemnify, and hold harmless the agency that issued the license, its chief officer or any of its employees from such claim, suit, or action.
179.
180. I understand that the acceptance of my application by the licensing authority does not guarantee the issuance of a license and that fees and costs are not refundable if denied. I further understand that if my application is approved and I am issued a license to carry a concealed weapon, that the license is subject to restrictions placed upon it and that misuse of the license will cause an automatic revocation and possible arrest and that the license may also be suspended or revoked at the discretion of the licensing authority at any time. I am aware that any use of a firearm may bring criminal action or civil liability against me.
181.
182. I have read, understand, and agree to the CCW license liability clauses, conditions, and restrictions stated in this Application and Agreement to Restrictions and to Hold Harmless.
