Data and information about the existing health care system in the Philippines

are gathered from several clinics and a hospital in Marikina City. According to

the analysis of the information, some medical institutions in Marikina City are

still implementing the outmoded system which the medical records of in-patient

are recorded on papers and index cards. All medical records such as medical

history, laboratory results, diagnosis, and etc., are recorded on a paper and

stored in a file cabinet. Clinicians can only generate diagnosis for out-

patients after referring to the medical history of the receiver. The out-

patient’s medical records are gathered from the out-patient itself through an

interview.
In some hospitals, they have an existing system specifically a database system

that stores the medical records of all patients admitted to their medical

institution. This system revolutionizes the old card catalogued file management

system of hospitals. All the medical records of their patients are stored in a

database that can be accessed by clinicians easily.
