4) Form

In the past I have been pretty lax with the forms I accept but this time around I am not going to do so. I am probably going to do first come first serve but that doesn't mean you get to do a shitty form. Please take your time with your forms and fill in all the prompts.

I accept google docs, pastebin and carrd. No commented forms please and thank you. Also aesthetic forms are not necessary, but you can add any aesthetics if you would like too.

Face claims can be of any nationality or ethnicity. I have decided to make this a multi apply fic.

Also no mary sues.

Almost forgot, this is likely going to be based in America or England (most like England because that is where I be from)
