Honestly, I think Millie would take the situation far, far, worse than Moxxie. You have the combo of

She'd still get all the tender loving care she needs from Moxxie, as he'd never leave her, but she'd need a lot more emotional support. I'd also suspect that Blitzo and Loona would be far more sympathetic towards her.

I feel like she'd be stuck for longer in the anger and depression cycles of the 5 stages of grief. I'd think Moxxie would more quickly adapt to the changes more so because his profession is based on firearms usage which is easier to adapt to, but Millie on the other hand would be far more stubborn in trying to lose the weight, even going to extremes like aggressive liposuction or dabbling in magic.
So long as she can be supported with a lot of TLC during her depression, she'll eventually come to accept the change.

I don't know about that, but she might be able to use her weight more effectively if she's strongfat. Basically be a cannonball.

Depends how the fat works, she'd 100% adapt to her new body given time and effort, I doubt she'd be as nimble as before but she'd find a way to use her new weight range to her advantage, she might have to learn how to adopt guns into her combat style though.

Might actually serve as a negative at first, she'd struggle to enjoy any positions with Moxxie. Again, they'd adapt but she'll likely be saddened she'll never be able to enjoy the original kind of s-x positions the pair partook in.

Of course I'm sure gluttony and lust have extra large sized straps for di-do, and I'm certain the pair would find a way for Moxxie to position himself to pleasure Millie.

it also depends on HOW big she gets. like if its just a moderate amount of chub, she'd probably not sweat it that much, but if it was as big as the fat moxxie drawings then that's when she'd probably start panicking and despairing

I wonder if turning to angels could help in theory, there are seven virtues as well. They'd have to find a way to draw on Temperance, but doing so would require pulling strings they dont have access to.

Moxxie fat levels of fat would, as you note, leave her distraught and maybe even teetering towards sui-dal thoughts initially.

Looking back I think the best hope you'd get is trying to use magic bulls-t powers to hide the fact you are fat. Of course only people like Stolas would have that since regular hellborn don't have access to magic.
Even then I'd argue trying to hide your massive fat gain would be a massive drain on you and leave you fatigued and worse off than just not bothering.

Considering Stolas' standing I'd be surprised if he didnt know some obscure spell or method to fix it or how to find someone who can. Dont forget hes pretty high up on the totem pole himself.

the answer boils down to if the fat disease can be cured with magic or not, i'd guess there is a magic cure but that it is obscure or hard to acquire

or there are only "workaround" cures to it like what you said with masquerading it, but it makes you feel much more drained like you have cinderblocks strapped to your back

Exactly. I think an outright cure would diminish the impact of the disease (it'd make a nice wham moment to think you found a cure but discovered its literally impossible) but there being some workarounds makes sense.
You could have a case of some highly ranked demon contracting the disease and them going out of their way to hide it, with the requirements to keep it hidden becoming harder and harder to maintain.

Of course I'm sure God could cure it. But if you, a demon, managed to convince God to come all the way down to heaven, and your only query was him curing your fat. I'm certain someone is going to shoot you the second God leaves.
