Dear Gerard, please find below my treatment plan:



Patient : Mister Gerard JARZEBAK

DOB: 26/05/1995



Diagnostic:

-lower jaw in retro position

-overbite 100%

- overjet 4mm

- class II left side

- in centric biting position no contact between upper and lower left



Treatment proposed:

Fonctionnal active treatment with Sodis apf aligner,

A series of aligner may be needed, but we will start with the first one Round expander EOA : 27 mm,

May be follow by E1A 29 mm, E2A 31 mm, E1C 33mm, so a total of 4 aligners.
