- TR Maggy now receives a damage boost as soon as CA makes contact with an enemy.
- TR Maggy now has significantly reduced starting stats (Compared to base Isaac stats: 0.75x damage multiplier, 0.9x tears multiplier, -0.2 speed).
- Cardiac arrest is now spectral.
- TR Maggy's damage boost and heart drop quantities are now determined by how many enemies in a row Cardiac Arrest has hit (~+1 damage for 1st, ~+0.5 for next 4, ~+0.25 for consecutive enemies).
- TR Maggy's damage boost now drains significantly slower.
- TR Maggy now starts with 3 heart containers instead of 4.
- Cardiac Arrest now only takes half a heart from the player.
- Bleeding has been completely revamped and is now a mechanic shared between CA and TR mag. You are no longer always bleeding, and instead, CA adds 1 full heart’s worth of bleeding on use. The number of bleeding hearts goes down in half-heart increments as you bleed through hearts. Mag also receives 1 full heart’s worth of bleeding on enemy damage (not on self-damage).
- Picking up extra hearts no longer gives stat boosts, and instead will add to a new "bonus heart" counter. If there are hearts present here, this counter will be drawn from when bleeding instead of your main heart containers, and if you hit a certain threshold of bonus hearts, you will be given a new heart container and your bonus hearts will be drained. Your bleeding hearts will also be drained by the same amount of bonus hearts lost to compensate.
- To accommodate for these major changes to the bleeding mechanic, her bleeding UI has also been overhauled to display more information including the number of bleeding hearts, the number of bonus hearts you have, and the number of bonus hearts required to gain a heart container
- When you run out of hearts to bleed through (on half a red heart), you will start to lose heart containers. If you run out of heart containers, you will die.
- While bleeding is active, TR Maggy now produces Anemic creep similar to how T Maggy does
- TR Maggy's bleed timer no longer resets when using CA.
- Cardiac Arrest damage boost is now a damage up instead of a damage multiplier.
- TR Maggy's CA damage boost magnitude is now influenced by her base damage; Despite the boost being higher, it will fully drain in the same amount of time.
- TR Maggy now gains a large but temporary speed boost upon taking damage (this includes using CA).
- Cardiac Arrest now creates a large creep pool where the player is standing on use.
- Usage of Cardiac Arrest is now considered self-damage, and will now trigger affected item effects accordingly
- Visually, TR Maggy has a brand new sprite, as well as blood tears.
- Birthright now adds the bleeding status effect to hit enemies in addition to the old effects.
