  9. These terms apply to all programs operated by Peralta Design (referred to as “PD Code Camp”) including but not limited peraltadesign.com These terms apply to all courses, options, and programs offered by Peralta Design from July 29, 2017.

  11. RELEASE FROM LIABILITY

  13. The participant has permission to engage in all activities. The parent/guardian gives permission to PD Code Camp, administer prescribed medications, and seek emergency medical treatment, including ordering of X-rays and/or tests. The parent/guardian furthermore gives permission to the professional physician to secure and administer treatment including emergency medical or surgical treatment and hospitalization if necessary.

  15. The parent/guardian will be financially responsible for any medical attention needed during the duration of the program or resulting from any injury received. The parent/guardian shall provide the sole insurance coverage for any medical treatment.

The parent/guardian agrees to release any records necessary for insurance purposes, with the understanding that every attempt will be made to contact the parent/guardian, or the emergency contact(s), before taking this action. The parent/guardian gives permission to Peralta Design to arrange necessary related emergency transportation for the participant.

I HEREBY WAIVE AND RELEASE Peralta Design, ITS OWNERS, AGENTS, PARTNERS, FACILITY PROVIDERS, AND EMPLOYEES FROM LIABILITY FOR ANY INJURY OR ILLNESS INCURRED WHILE AT CAMP, RESULTING FROM ORDINARY NEGLIGENCE. I UNDERSTAND THAT THERE IS RISK OF INJURY TO THE PARTICIPANT AS A RESULT OF CAMP AND ACADEMY ACTIVITIES, AND KNOWINGLY AND VOLUNTARILY ASSUME ALL RISK OF SUCH INJURY. HOWEVER, I UNDERSTAND THAT I AM NOT RELEASING Peralta Design ITS OWNERS, AGENTS, PARTNERS, FACILITY PROVIDERS, AND EMPLOYEES FROM GROSS NEGLIGENCE, RECKLESS CONDUCT OR INTENTIONALLY TORTIOUS CONDUCT. TO THE EXTENT THIS RELEASE CONFLICTS WITH STATE/PROVINCIAL LAW GOVERNING RELEASES, THIS RELEASE IS TO BE GIVEN THE FULLEST FORCE AND EFFECT PERMITTED UNDER STATE OR PROVINCIAL LAW. IF THIS RELEASE IS DETERMINED TO BE INVALID UNDER THE LAWS OF THE GOVERNING STATE OR PROVINCE, THEN THIS RELEASE SHALL BE STRICKEN FROM THIS CONTRACT, BUT ALL OTHER TERMS AND CONDITIONS OF THIS AGREEMENT SHALL REMAIN IN FULL FORCE AND EFFECT.

  21. STATEMENT OF CONSENT/ARBITRATION CLAUSE

  23. By authorizing the participant to attend Peralta Design programs, the parent/guardian must agree to the statement of consent and the participant code of conduct. The information we collect is CONFIDENTIAL and is used to aid our staff, the Camp Director and Instructors in assessing crucial information regarding the participant.

In the registration, you will need to submit information about the participant including allergies, illnesses, medications, dietary restrictions, immunizations, emergency contact information, gender and date of birth. We ask for a Primary Contact Person in case of emergency (in addition to a Secondary Contact Person) and information concerning these parties—such as name of contacts, phone numbers, email, and address.

  27. Participants will only be admitted to the PD Code Camp programs with a named physician. We ask for the physician’s name, along with the physician’s contact information. Additionally, it is required that you provide the medical insurance carrier and the policy number or social security number of the participant. If the participant is not covered under a health insurance plan, or if you do not wish to provide Peralta Design with your medical insurance information, you agree to accept all financial liability should your child become injured while at one of our camps, and release and indemnify Peralta Design from any such financial responsibility.

  29. If your participant should need to be restricted from any activity, you will be asked to inform us with the specifics. You further certify that he/she is in good health and able to participate in the program activities. You authorize the staff to attend to health problems or injuries your son/daughter/ward may incur while attending our programs. You acknowledge that you are responsible for medical expenses due to your son/daughter/ward’s illness or injury.

  31. ALLERGIES/DRUG REACTIONS and Management of Reactions including NUTS. You will be asked to chronicle any known allergies. You will be asked if the participant is bringing an EPIPEN to camp. If so, you will be asked to verify the prescription and bring 2 (two) EPIPENS that are unexpired to the program(s); one for staff and one for your participant to carry on their person. We cannot admit your participant without 2 (two) EPIPENS; this is for your participant’s safety. You may be asked for immunization information.

  33. You will be asked to enter all known illnesses or conditions which the participant has or has had including illness, allergies, and drug reactions. You will be asked to fill out a list of prescribed medications your son/daughter/ward MUST take while attending camp. You acknowledge that the medicine and/or vitamins are to be in the possession of the Camp Director or other designated staff, delivered to the staff in original containers that identify, in English, the prescribing physician (if a prescription drug), the name of the medication, the dosage, any side effect, and the frequency of administration, and will be dispensed as instructed by you.

  35. Exception: INHALERS AND DIABETIC TESTING KITS. Any discrepancies may result in long delays at check-in and may require the cancellation of the session, without a refund. Our program staff needs your full cooperation. Thank you for understanding. You also understand that no one at Peralta Design is authorized to give any participant over-the-counter medication without written permission. And, in some states it is required that both the guardian and family physician provide written approval. In all cases, our staff are not professional nurses or doctors, and have not been trained in medicine.

  37. You understand that if your participant is taking medication, all medications must be brought in the original container. The original container must identify (in English) the prescribing physician (if a prescription drug), the name of the medication, the dosage, any side effect, and the frequency of administration. Participants needing injections (insulin, hormones, etc.) will need to self-administer the medication under observation of our staff. Peralta Design staff is not trained in this area. Upon arrival, all medication must be placed in the care of our designated staff.

  39. Our staff will only administer the dosage as listed on the container. Please provide a physician’s note if your participant’s dosage differs from what is listed. You understand that any discrepancies may result in long delays at check-in and may require the cancellation of the program, without a refund. Please refrain from bringing non-essential vitamins and/or supplements. Our staff kindly appreciates your full cooperation.

  41. You understand that it is the responsibility of all parents/guardians to retrieve their participant’s medication from the staff at the end of the session. If medication is not picked up within seven days, all medication(s) that is left behind after that grace period will be disposed of. Should Peralta Design dispose of any medication pursuant to this policy, Peralta Design shall not be responsible for any damages or liabilities related in any way to such medication, including, but not limited to, replacement cost of the medication and any damages resulting from participant’s failure to administer such medication. PD Code Camp, by law, is not allowed to mail medication, so please be sure to retrieve all medication upon checking out.

  43. If the participant has a dietary restriction or food allergy of any kind, you will be asked to provide your own lunch.

  45. COMMUNICABLE DISEASES DISCLAIMER

  47. Communicable diseases, also known as infectious diseases or transmissible diseases, are illnesses that result from the infection, presence and growth of pathogenic (capable of causing disease) biologic agents in an individual human or other animal host. Transmission of these biologic agents can occur in a variety of ways, including direct physical contact with an infectious person, consuming contaminated foods or beverages, contact with contaminated body fluids, contact with contaminated inanimate objects, airborne (inhalation), or being bitten by an infected insect or tick.

  53. The best protection against most communicable diseases is vaccination. We encourage all parents / guardians to have their participant receive childhood vaccinations to protect them against illness. The Centers for Disease Control (CDC) and the World Health Organization (WHO) provide valuable information on communicable diseases and vaccinations.

  55. Lost, Stolen, or Damaged Property. You will be responsible for personal or property damage incurred by your son/daughter while attending a program. Peralta Design is not responsible for personal belongings lost or damaged by casualty, theft, laundry, etc. You agree to accept full responsibility, financial or otherwise, for the conduct of your participant—including damage to equipment, or facilities. Peralta Design does not owe a duty to protect its participants from theft or damage to property. You will not hold Peralta Design responsible for any lost, stolen, or damaged personal items. If any theft of or damage to your son / daughter property should occur, or in association with the program, you understand that Peralta Design will not be held responsible.

  57. You understand that all images, testimonials, photos, video, and audio taken at or in connection with Peralta Design are the sole and exclusive property of PD Code Camp. As a condition of participation, you must understand and authorize that photos may be taken of your participant. You also understand and agree that said content may be used by Peralta Design in promotional materials, marketing collateral, and online media. These images, testimonials, photos, videos, and audio may be shared and used by corporate partners, the media, or other organizations who work with PD Code Camp. You understand that PD Code Camp, its owners, agents, partners, facility providers, and employees will not be held liable for damages and injuries associated with said publishing, including any and all claims based on negligence.

  59. Press Visits . Please note that if Peralta Design is contacted by the press, a participant’s full name may be used in print, radio, TV and other mediums. We may not have the opportunity to contact all families for permission before the press arrives. Therefore, we have created a blanket policy. We thank you for understanding.

  61. You understand that all projects and work created while at Peralta Design may be used by Peralta Design in promotional materials, online, and other print media. These projects may be shared and used by corporate partners, the media, or other organizations who work with PD Code Camp. You agree that any digital movie created during the Peralta Design course may be submitted to a film festival by Peralta Design and/or any of the participant(s) in the group that created the movie. You understand that PD Code Camp, its owners, agents, partners, facility providers, and employees will not be held liable for damages and injuries associated with said publishing, including any and all claims based on negligence.

  63. You understand that movies played at any of the programs run by Peralta Design are rated PG and PG-13 and selected at the sole discretion of PD Code Camp. You understand that video and computer games used at any of the programs run by Peralta Design are selected at the sole discretion of Peralta Design staff.

  65. You understand that some Peralta Design game development courses require creation or use of an online account during instructional time, or will require an online account to be created for access to games and networked gameplay.

  67. Games Rated “T” and “M”—Our Responsibility: Peralta Design has adopted the policy of allowing the use of select games rated “E” for Everyone, “T” for Teen, and “M” for Mature to be used as part of the curriculum, and available for rec-time or down-time, including aftercare.

Participant Projects: You understand that Peralta Design makes a reasonable effort to send each participant home with a working game or project, or posts the participant’s project to a project website. However, there will be instances when the game or project will not work on an individual’s home system. In these rare cases, Peralta Design does not take responsibility for non-functioning/non-compatible/non-complete games or projects. Peralta Design staff will provide each participant with their completed project at the conclusion of each session. It is the responsibility of the parent/guardian and participant to test the participant’s project on the Peralta Design computer in the lab before going home at the end of the session. Refunds will not be issued for non-functioning/non-compatible/non-complete projects.

  71. Sign-Out Policy: All participants must be signed out of our care by the guardian or authorized pick-up person. In order to pick up your participant from camp, only those named as additional authorized pick-up persons (and who show a valid picture ID, Driver’s License, or Passport)—will be eligible.

  73. You understand that Peralta Design reserves the right to cancel a course. If a course is cancelled, you understand that you may enroll your son/daughter/ward in another course. If one is not available in the local vicinity for the same dates, a refund will be issued for all monies paid to Peralta Design for the cancelled course. You understand that if Peralta Design is forced to close a course based on a recommendation by the local Health Services Department (in the case of pandemic flu, etc.) or due to other acts of nature, no refunds will be issued. The tuition paid for the cancelled course can be applied to a voucher that can be used during that same season (as space permits) or will be carried over to the next season.

ON BEHALF OF YOUR SON/DAUGHTER, THE PARENT/GUARDIAN HEREBY RELEASES Peralta Design., ITS OWNERS, AGENTS, PARTNERS, FACILITY PROVIDERS AND EMPLOYEES FROM LIABILITY (INCLUDING CLAIMS BASED UPON NEGLIGENCE) FOR DAMAGE OR INJURY TO YOUR SON/DAUGHTER/WARD OR DAMAGE OF ANY PERSONAL PROPERTY, ACCEPTING YOURSELF THE FULL RESPONSIBILITY FOR ANY AND ALL SUCH DAMAGE OR INJURY WHICH MAY RESULT DIRECTLY OR INDIRECTLY FROM ANY NEGLIGENT ACTS OR ACTIVITIES ASSOCIATED WITH Peralta Design HOWEVER, YOU UNDERSTAND THAT YOU ARE NOT RELEASING Peralta Design, ITS OWNERS, AGENTS, PARTNERS, FACILITY PROVIDERS, AND EMPLOYEES FROM GROSS NEGLIGENCE, RECKLESS CONDUCT OR INTENTIONALLY TORTIOUS CONDUCT. TO THE EXTENT THIS RELEASE CONFLICTS WITH STATE/PROVINCIAL LAW GOVERNING RELEASES, THIS RELEASE IS TO BE GIVEN THE FULLEST FORCE AND EFFECT PERMITTED UNDER STATE/PROVINCIAL LAW. SHOULD ANY PART OF THIS CONTRACT BE FOUND INVALID OR NOT ENFORCEABLE BY A COURT OF LAW, THEN THE REMAINING PORTION SHALL CONTINUE TO BE VALID AND IN FORCE.

  77. YOU UNDERSTAND THAT THERE IS RISK OF INJURY TO THE NAMED PARTICIPANT AS A RESULT OF ACTIVITIES, AND KNOWINGLY AND VOLUNTARILY ASSUME ALL RISK OF SUCH INJURY. BY ENTERING, IT CERTIFIES THAT YOU, THE PARENT/GUARDIAN, AGREE TO THE TERMS STATED WITHIN, AND WILL TAKE THE TIME TO ENSURE THAT ALL INFORMATION IN THE REQUIRED FORMS WILL BE UP-TO-DATE AND ACCURATE TO THE BEST OF YOUR KNOWLEDGE, PRIOR TO ATTENDING THE Peralta Design PROGRAM.

  79. THIS AGREEMENT CONTAINS A BINDING ARBITRATION PROVISION WHICH MAY BE ENFORCED BY BOTH PARTIES.

  81. ARBITRATION IN LIEU OF LITIGATION: You agree that any dispute arising out of or relating to this Agreement, your participant’s participation at Peralta Design, or otherwise arising between the parties, including, without limitation, any participants statutorily created or protected rights, as permitted by applicable state/provincial or federal laws, may be settled by arbitration in the state/province and county in which the camp is held, in accordance with the Commercial Rules of the American Arbitration Association, and judgment upon the award rendered by the arbitrator(s) may be entered in any court of competent jurisdiction. The prevailing party in the arbitration shall be entitled to recover expenses including costs and reasonable attorney’s fees associated therewith. Should any part of this contract be found invalid or not enforceable by a court of law, then the remaining portion shall continue to be valid and in force. You hereby acknowledge that you understand the terms of the ARBITRATION AGREEMENT, and agree to comply with all of its terms and provisions.

  83. Peralta Design CODE OF CONDUCT

  85. Peralta Design has the right to dismiss any individuals if their actions or attitudes are deemed detrimental, as set forth below in the Peralta Design Code of Conduct, and that there shall be no refunds issued for such dismissal.

  87. You agree that you have read through, understand, and agree to the Code of Conduct. You further agree that you will review the Code of Conduct with your participant(s) that is/are participating in a program run by PD Code Camp. As the parent/guardian of a participant attending a Peralta Design program, you understand that your participant must act in a way that is in keeping with the Code of Conduct. You also understand that your participant can be dismissed from a program if his/her actions or attitudes seem to be harmful to the atmosphere or to other participants, in the opinion of Peralta Design staff (center director).

You understand that your participant can be dismissed, without a refund, from a program for the following reasons: drug/alcohol/tobacco use; viewing pornography or any inappropriate material on the Internet; inappropriate language (swearing, using bad words, racial slurs, etc.); physical violence or threats of physical violence against other participants or Peralta Design staff; damage to facilities or Peralta Design equipment; Stealing property of others; inappropriate use of equipment; sexual harassment of other participants or Peralta Design staff; Internet hacking; or bullying.

  91. If possession of illegal substances or weapons is found, participant(s) will be dismissed from the program immediately. Personal pets or animals are not allowed to be brought or kept at camp. Participant (and therefore parent/guardian) is responsible for any personal items brought to and loaned. Participant promises to respect all Peralta Design staff members and other participants while treating the facilities and equipment with great care.

  93. Bringing electronic devices, including smartphone, laptop, digital camera, handheld console, etc. We do NOT require participants to bring any devices to the camp. Additionally, we understand that parents may want to communicate with their students via cell phone. Should the participant bring a personal device to the program, please note:

  95. Peralta Design is NOT responsible for lost, damaged, or stolen equipment—participant and/or parent/guardian assume all risk. Personal devices may NOT be used during instructional times—unless they are approved by the instructor. We recommend that personal devices be locked in a dorm room or backpack. Cell/smart phones can be kept on hand but must be in silent mode during instructional times. Accessing the Internet via a local Wi-Fi hotspot at the program location (i.e., university campus) is the sole responsibility of the participant. Participant and parent/guardian must comply with all providers terms & conditions, and assume all financial risk. Peralta Design does not facilitate Internet access for non-Peralta Design devices. Viewing inappropriate content (including sexual content, inappropriate violence, racism, bullying, etc.) online may result in immediate dismissal from a program, without a refund.

  97. Recommended: Participants have access to tons of technology while at PD Code Camp. Feel free to leave personal electronic devices at home. Certain filtering software is placed on Peralta Design computers, and this may not be the case with a personal device. Additionally, studies from the American Camp Association have shown that students who leave their devices at home gain a greater sense of independence. Plus, there is always email.

Parent/guardian and participant understand that Peralta Design reserves the right to dismiss participant from the program without any prior warning for violating any of the terms of the Code of Conduct. You acknowledge that upon staff observation of your participant at the Peralta Design program, the Director may determine that the program is not a suitable and/or productive environment for your participant. In this case, Peralta Design may dismiss the participant and no refund will be issued. Peralta Design will exhaust every opportunity for an excellent relationship with every participant. However, in some rare cases, Peralta Design may not be a compatible environment for every participant.

  101. PROMOTIONAL CODE DISCOUNTS

  103. Utilize a valid Promotional Code and receive a discount (limit one per participant). Peralta Design will apply the discount promotional code with the greatest amount to each account. Discount amounts vary depending on program course purchased. Note that some Promotional Codes require that the session balance be paid in full at the time of registration to receive the promotional discount. All promotional codes must be submitted at the time of registration. The total discounts received cannot exceed the total cost of the products purchased.

  105. REFER-A-FRIEND: DETAILS

  107. There is no limit to the number of friends a client can refer.
  108. Earn $25 credit for each new participant you refer who attends PD Code Camp. The referred participant must be a new participant and cannot have previously attended Peralta Design programs.
  109. Refer 5 friends and all 5 join a camp you will get a free computer at the end of the summer. In order to be eligible, you must have referred a new participant prior to the date your friend starts camp.
  110. The new participant MUST enter their Referral Code at the time of registration. Codes will not be accepted after the participant is already registered.
  111. You can refer as many eligible participants as you like, quickly and easily through My Account.
  112. Referral codes can only be used once per participant. Please note that Peralta Design will credit the referral based off of the referral code that the new client enters at the time of registration.
  113. The Refer-a-Friend program does not apply to siblings.
  114. Participants may not refer each other to both qualify for the Refer-a-Friend discount.
  115. For each referral, you will receive a credit to your account.
  116. The credit will be applied within 1 week from the time the referred client registers, or sooner. If referred friend cancels his/her session, the credit will be removed from the customer’s account.
  117. Peralta Design reserves the right to update or modify these terms and conditions on the website at any time.
  118. TUITION INCLUSIONS

  120. Program Tuition includes all state/province and federal taxes, adult supervision, instruction, curriculum materials, camp T-shirt, and recreational activities.

  122. Camp begins at approximately 9am and concludes at approximately 3:30pm (Monday-Friday).

  124. Students enrolled with aftercare may check-in as early as 7:30am and stay as long as 6:00pm.

  126. CAMP LUNCH PLANS

  128. Lunch is provided for all camper (except those with allergies of any kind), bringing a brown bag lunch is acceptable We ask that families who are packing lunch send non-perishables that do not require refrigeration. Lunch is facilitated through licensed food service providers.

  130. YOUR CREDIT CARD INFORMATION AND TRANSACTIONS ARE SECURE

  132. For your convenience, you can pay by credit card. we will not keep your credit card information on file, For your security, your credit card security number is not stored in our system.

  134. We use state-of-the-art Secure Socket Layer (SSL) encryption technology to safeguard and protect your personal information and transactions over the Internet. Your information, including your credit card information, is encrypted and cannot be read as it travels over the Internet.

  136. BACKGROUND CHECKS

At Peralta Design, we require criminal background checks for Instructors, Directors, Managers, Aides, and corporate staff who will be onsite for any duration of time other than end of session Friday family events at our locations. We take the safety and security of our participants very seriously and require criminal background checks for anyone authorized by Peralta Design to be in contact with the participant for the duration of their stay.

  140. SOCIAL NETWORKING DISCLAIMER

  142. Peralta Design provides several opportunities for social networking for both participants and staff on sites such as Facebook, Twitter, Flickr, and YouTube. These sites are not affiliated with Peralta Design and offer their own individual social networking services. Please read the following Terms and Conditions carefully, as well as the Terms and Conditions of the sites in which Peralta Design has created a forum (“Group”). These Terms and Conditions are a legal agreement between you and Peralta Design and apply to you whether you are a visitor to these sites or any site with an official Peralta Design affiliation.

  144. Peralta Design is a member of several pre-existing sites (as mentioned above). There are, however, portions of the website that include areas where participants can post submissions. Any of the above-mentioned “Sites” (or other similar sites) have their own distinct rules and regulations. Peralta Design reserves the right to take action to remove any content deemed inappropriate by the sites or by Peralta Design standards. Peralta Design will not be held liable for any loss of content or disagreements that may arise between the individual social networking site and the user.

  146. You understand that by registering for a Peralta Design program, your participant(s) may access and upload content to social networking sites. In order to access certain features of the social networking sites or to pages on PD Code Camp, the majority of these sites require that the user open an account with them. Please note that these sites have their own individual Terms and Conditions that must be followed. Age requirements are outlined within each Site’s Terms and Conditions. You hereby authorize your participant to access social networking sites while at camp and create an account if they choose to do so and if they meet the requirements listed by each site to create an account.

  148. Interaction with other users:

  150. Peralta Design is merely providing a medium in which to socialize online with fellow participants. Users are solely responsible for interactions (including any disputes) with other Members and any volunteers that may advise and assist participants with projects and activities via your use of the Site and Services. You understand that Peralta Design does not in any way screen Members or review or police: (i) statements made by Members in their Member Submissions or the Member Submissions in general; or (ii) statements made by Users or any information a User may provide via the Site or Services. You understand that your participant(s) is solely responsible for, and will exercise caution, discretion, common sense, and judgment in using the various Sites and Services and disclosing personal information to other Members or Users. On behalf of your participant(s), you agree that they will take reasonable precautions in all interactions with other Members, particularly if they decide to meet a Member offline or in person. Your participant’s use of the social networking sites with which Peralta Design is affiliated, their services, Content and Member Submissions is at your sole risk and discretion and Peralta Design hereby disclaims any and all liability to you or any third party relating thereto. On behalf of your participant(s), you agree that they will not harass, threaten, intimidate, bully, stalk, or invade the privacy of any individual in connection with your use of the social networking sites with which Peralta Design is affiliated and their services, whether or not an individual is an Peralta Design Member; and you further agree not to advocate such activities or to encourage others to engage in any such activities. On behalf of your participant(s), you agree they will not give their social networking information to a Peralta Design staff member. You and your participant(s) should also be aware that under no circumstances are Peralta Design employees allowed to give personal contact information for social networking sites. This must be arranged by the participant’s parent/guardian through the Human Resources Department.

COPYRIGHT & INTELLECTUAL PROPERTY POLICY- NO UNAUTHORIZED USE: You agree that you and your participant will not use the social networking sites to offer, display, distribute, transmit, route, provide connections to, or store any material that infringes copyrighted works, trademarks, or service marks or otherwise violates or promotes the violation of the intellectual property rights of any third party. Peralta Design. has adopted and implemented a policy that provides for the termination in appropriate circumstances of the accounts of users who repeatedly infringe or are believed to be or are charged with repeatedly infringing the intellectual property or proprietary rights of others.

  162. Peralta Design reserves the right to update or modify these Terms and Conditions on the website at any time. We reserve the right to cancel a course if participation numbers are not adequate: In the case a suitable Peralta Design replacement week is not found, a full refund will be issued. On rare occasions, the software and hardware we use at our programs may change without notice, and we will strive to find suitable software and hardware alternatives.

  164. CANCELLATION BY CAMPER / PARENT (REFUND POLICY)
  165. By submitting this registration you are agreeing that you understand the that all camp payments are refundable
  166. before June 1st less the registration and curriculum fees ($99). After June 1st and upon receipt of written
  167. cancellation not less than fourteen (14) calendar days prior to the start of the booked camp session in question
  168. will be refunded 50%. Beyond these refund deadlines, there are NO REFUNDS for any reason.

Yes, I certify that I am the cardholder listed above and I authorize Peralta Design and its registration service provider to charge my credit card. I understand that in the case(s) stated above where I am not the Legal Guardian of the student, the Legal Guardian must also agree to the Terms & Conditions in order for the student(s) to participate in any Peralta Design (PD Code Camp) programs.
