Ingredients for Potion of Transformation
-1 dose purest midnight tears
-1 dose venom from a wyvern killed within the last 60 days
-1 liter unicorn blood
-1 liter blood from a vampire lord or vampiric thrall (should be noted that the former increases the chance of the ritual's success, but I'll make do with what I must in a pinch at my own peril)
-The heart and 1 liter of blood of a humanoid killed within the last 24 hours (can be same as sacrifice)

INSCRIPTIONS UPON THE PHYLACTERY ARE AS FOLLOWS

-The other methods Azoch mentioned to me are guaranteed successes, but cannot be considered. If an innocent's life is what is needed to erase the risk of failure, then I'm willing to gamble my soul.

[ADDITIONAL NOTES FOR PEOPLE WHO WANT TO REFERENCE THIS:

-The phylactery must be specifically crafted for the ritual; you can't use an existing item. It also has to be of a fairly high monetary value, and the craftsperson who creates it doesn't need to specifically know it's going to be used as a phylactery.
-You can, however, incorporate existing items into the finished product as long as it's not the entirety of it.
-The phylactery must be some sort of object that can function as a container, such as a jar, locket, scabbard, or box, or, failing that, be hollow on the inside.
-Each lich knows the means by which their phylactery can be destroyed upon achieving lichdom, but like hell are they gonna tell you.
-The phylactery must incorporate some aspect of the lich-to-be's personal history, identity, or personality in order to properly house their soul.

-Becoming a lich SPECIFICALLY has the ritual be powered by someone's drive and desperation to achieve immortality. This is why more depraved acts of cruelty have a higher success rate, but you can literally kill an asshole if you don't want to be the asshole. It's just that someone willing to do the unspeakable is likely to have more battery power for this to work.
