- yet another style of presentation again, actually had this one in the works for a couple of weeks. the greentext itself can definitely stand on its own, but the paper (or at least the first page excerpt) really supplements it, though i do acknowledge that the article itself absolutely gets way in the weeds since it came across really awkwardly when i tried writing it in a less stringent way.

- >Anyway, today I’ll be discussing a fairly recent paper that was published in the Journal of Neurological Sciences just last month, “Butting Heads with Concussion in Horned Species” by Awassi et al., which I believe could have significant ramifications -- pun not intended – on clinical assessments for concussion in horned mammals.

- >This is actually a first of its kind paper, in that previous research has actually not investigated concussion frequency in specifically horned species examined in this paper, likely in part because there’s this widespread belief (even amongst medical professionals) that horned mammals are less susceptible to them.
- >There have been many a paper published on concussions, the hallmark analysis by Reynard et al. back in 2009 comes to mind, but that and all of the currently established literature casts a fairly broad net in terms of species studied, rather than focusing on said horned species. - >… and to start things off, let’s all make sure we’re on the same page. The Introduction of the paper defines exactly what a concussion is, which I’m sure we all know is a traumatic injury of the brain that – though milder than say, something causing actual intracranial hemorrhage – still causes disruption of various functions of the brain, manifesting with well-known symptoms like mood swings, impaired balance, disorientation…
- >Concussion diagnosis is something you really want to nail, since as we all know, a second impact before full recovery from the first can be absolutely catastrophic and fatal – mortality rate of about 50%.

- >…getting into the Methods section now, looking at the way they’ve set this paper up? Pretty damn good. Inclusion and exclusion criteria they use to systematically look at papers and their datasets are very reasonable here. They make special note that the papers that they included were granular enough to differentiate data points by species, but let’s also look at their statistical analyses…
- >… and after all that is said and done, they’ve pulled 793 horned mammals into this massive conglomerate from 23 papers in the Data section, though this isn’t quite an even distribution. About 25% of these came from one paper alone, itself limited to studying patients from two specific clinical sites in the Meadowlands, which could raise questions about how generalizable these results might be. Furthermore…
- >…we have finally arrived at the Discussion and Conclusion. The big thing they really emphasize here is that your level of suspicion for a concussion in a horned mammal should be exactly the same as any other species when they come in with these symptoms after an impact to the head. Do NOT alter your differential diagnoses just because of those big ol’ honkin’ things on top of your patient’s head, though this is obviously more easily said than done. Fortunately, they do suggest *some* objective scales that you can use when assessing mammals that can aid in decreasing bias…

- >… and with regard to limitations of the paper, they do acknowledge that it is somewhat limited in scope, given that many of the papers that they analyzed were subsequently excluded due to lack of delineation of species of their subjects. This resulted in easily hundreds or even thousands of potential horned mammal data points being thrown out, which could have affected the conclusions of this paper, but they had to work with what they got. Obviously, further studies should be done in this subset of mammals, given that…
