The user requirements state that the new database I am developing, needs to hold a minimum of five doctors, and about 600 patients which are currently on a paper and spreadsheet system, my database will transfer all the current data to an online electronic system, the database system will have an increased number of patients and doctors, the management team have asked me to upgrade from there old system to a new upgraded system which will allow the doctors and managers to easily access and add new patients / doctors with no hassle, the database will need to be easy and simple to understand to allow time for staff to become confident using my new database system, and software, my database also requires to have a limited risk to the patient safety.

Other user requirements which are necessary, is to have a minimum of at least two tables, which consist of 50 patients records and five Doctors on each of these tables and also have names, sizes, formats, and also datatypes, the database will require at least one working One-to-many relationship between the tables, in my database I am required to have validation procedures and verification some validation I will include will be for the name field which will only allow a certain amount of characters, the new database system will require two data-entry forms which will allow patients to be added to the new database and also a data-entry form which will allow for the new appointments to be booked between the doctors and patients.

The user requirements which have been set state that I will need to also include at least find queries which will extract meaningful information, the managers also have stated that I will need to create three reports to present information some of these reports will present information in a clear table for how many patients are ill with a cold or how many patients have gotten an appointment on XX/XX/XX date.
