  6. We understand unification has created unique situations where authority can be complex.

  8. We are curious who has the authority to conduct surprise inspections of FedEx North Salt Lake.

  10. We have tried multiple times over the last two years to report serious noncompliance.

  12. It is our hope that our messages were simply lost to a overloaded system.
  13. Our serious concern that Local Authorities in North Salt Lake have their interests tied to the political and finical loss that could occur, maybe they just didn't have the authority.

  15. However, or whatever the case; the internal audit of the same individual employee constantly checking the extinguisher levels monthly has never discovered the many ongoing serious fire system component failures.

  17. 1st and foremost an inspection and certification of the CO2 system should be the highest priority.

  19. There is no documentation available to employees or posted that explain the risks.
  20. You can watch from the public road, on a nearly monthly basis Sr Managers conduct an open grill cookout right next to the CO2 air intake systems.
  21. Where they have 'accidentally' filled the entire warehouse with smoke and fumes more than once. Where minor asthma attacks occurred, and employees were sent home with no documentation.

They self-built ‘out buildings’ over the summer where they disconnected the CO2 monitor systems entirely for months before retrofitting them back and it is our belief they have never worked properly since. We also are doubtful the increase in parking areas and addons can keep up with the fumes, drivers idle daily for extended periods of time. Where employees open garage doors or run outside to try and catch a breath as drivers dump unknown substances in the intake to jumpstart or ‘cold start’ motors spewing noxious fumes in the warehouse that are observable from the mezzanine in another part of the warehouse at least 100 yards away.
  23. They even conducted open propane grilling inside of the warehouse one day it was snowing too bad.

  25. 2nd and what we have reported by phone, by web-form and emails to NSL officials.

  27. Over 18 months ago staff members modified and bypassed the smoke detectors and required air circulation fans in all the staff bathrooms.
  28. We would also request a test and certification of those systems.
  29. Employees bring in e-cigarettes and vapes and use them in trailers and bathrooms creating an unimaginable risk if a battery were to explode. Managers bring in cellphone chargers and leave them unattended, we doubt the electrical systems can handle this unplanned load as circuits often break and cameras or computers stop working.

  31. We believe that other systems have been tampered with; we had multiple ‘false alarms’ where we were told to keep working and not allowed to evac with the alarm system blaring.
  32. We hope this isn’t lost and something can be done to protect the public and enforce the laws.
