Ontario Identification card missing

To whom it may consern

I hope this message finds you well. I am writing to you in light of a missing Ontario Identification Card that was sent from the:
Ontario Disability Support Program @ 119 King St W, Hamilton, ON L8P 4Y7 office to the:
Ontario Disability Support Program @ 351 Preston St Room 210, Ottawa, ON K1S 2T7 Office

I stopped by there today in person at 9AM-9:45AM Tuesday November 14th and was the 005 Number called when i spoke to the attendant and provided my name they were unable to locate my mail. I will be contacting the Service Ontario Or service canada in regards to my missing card. Thank you have a good day.
