Potential prize winner (or parent or legal guardian if potential prize winner is considered a minor in the legal jurisdiction of his or her residence) will be required to execute and return an Affidavit of Eligibility/Release of Liability and, if applicable, a completed IRS W-9 form within two (2) business days of the date of issuance of such documents. Potential prize winner’s failure to return all required forms in this two (2) business day time period may result in the potential prize winner being disqualified and an alternate winner may be selected at Sponsor’s sole discretion.
