Max out Pestilence
Max out Perpetuity
Max out Toxic Transmission
Get Discharge
Max out Endless Decay

Transplant:
Max out Violent Emergence
Max out Acolyte's Fervor
The rest of the points don't really matter, get them in any order

Reaper Form:

Death Seal:
Max out Entreat the Damned

Acolyte Passives:
Fill out the Acolyte tree as shown in the planner, skipping Crimson Gluttony. Take Crimson Gluttony when you equip a Plaguebearer

Gear/Idols:

Prioritize poison chance and cast speed. Chitin Dagger is a great choice if you don't have a Plaguebearer
Get capped Endurance ASAP. This is one of your most important defenses.
Get HP everywhere you can and resists/crit avoidance as needed. Woven Flesh is a good option early.

On idols, prioritize poison chance and transplant cooldown.
