Betty and Medea remain silent as you begin to head towards the Basement, though both of them easily keep pace with you, especially Medea, who after being blessed by Alítheia has become quite physically powerful for a Caster. Sure she's not going to win any strength contests compared to other Servants, but her strength is still very impressive for a Caster. As she is now, shes' unlikely to be defeated by Rin with her martial arts skills, the physical difference between the two of them would simply be too high even if Rin were to reinforce herself
