Location of the Realms

Yggdrasil - Norway, Oslo. The entire town was uprooted by a massive tree, killing thousands but nature has reclaimed this territory wholly, and this tree serves as a pathway to the realms of the norse gods. http://www.deviantart.com/art/Map-of-Yggdrasil-Nine-Worlds-291037202

Rai - The Rai is a mysterious place somewhere in the vast reaches of Russia, that is a heavenly paradise of where it is always nice, never cold, and filled with immortal spirits.

Slavic Hell - their hell is a vast, empty tundra where people are stuck for the rest of their lives, starving and freezing to death but never dying.

The Nine Layers of Hell - Jerusalem, Israel. This Holy City had so many slaughtered in the name of 'good' that it has now become the exact opposite. Heaven -  Jerusalem, Israel. The Angels here have practically soundproofed the underworld that lies beneath their holy city, in an attempt to keep this city sacred and preserve it, as well as to prevent Armageddon. They tend to keep the peace in the city, and try to keep the demons down below.

Diyu (Ten Courts of Hell) - Beijing, China. The city is overrun by oni and demons alike, torturing and murdering essentially anything that treads foot into the city, and the terrain has deformed vastly and developing it's own unique areas that are almost impossible to get through, showing endless torture in the ways of the eighteen layers of hell.

Tian (Chinese Heaven) - Shanghai, China. Essentially the same but with a bunch of immortal, very nice monks just chilling in paradise.

The Mayan Cenotes - Yucatan, Mexico. All of these caves end up leading to the same place: The underworld. Wonderful, right? They are beautiful, eerie caves with calm flowing water within them that seem to not even move.

Sumerian Hell - Victorian Falls, Zambia. Unfortunately, the sumerian deities were kicked out of the mesopotamia regions, however they managed to get quite a bit of Africa! Now their hell is right under the Victorian falls, where they have essentially just a vast, endless dull world similar to your own, but seemingly without the disruption from 100 years ago distorting the world. Just mindless zombies living a dull life using technology.

Sumerian Heaven -  Mt Kilimanjaro, Tanzania. Similar to Mount Olympus, but larger.

Mount Olympus - Shared by the Romans and the Greek gods alike, the only time they're ever actually to get along being so similar. Except for the fact that the Roman gods own half, and the Greek gods own half. They try to stay away from eachother.

The Underworld - The Entrance is the Acheron, of which unfortunately has been flooded by the river styx, causing all life around it for a fair ways to die, along with Cerberus having a bit more freedom, able to go slightly outside of the underworld.

The Pyramids - These are simultaneously the entrance to heaven, and hell. The top of the pyramids leading you to the gates of Aaru, the Egyptian heaven, and as you enter the tombs of the ancient phaorahs, you will find a passage deep into Duat, the egyptian hell. In Duat, it would actually be very similar to Egypt, except for you'll have to fight through ravenous spirits to escape once your in, and Anubis shall weigh your heart, or if you have entered their alive, let you leave.

Native Americans already live in their own form of heaven: the hunt. When they die, they are believed to be reincarnated back into one of their tribes or as an animal.

Karnataka - Nakara and Svarga lie here. A quiet little bit of ancient ruins, 25 kilometers in area. Once someone in their territory dies, they are sent underground here for temporary torture before being pumped back into the world as an infant or animal of some form. Or, people live above ground in paradise, unaware of the land below until it is their time to be reborn once more as someone of high status.

Japanese - Every family has their own temple dedicated to their ancestors, and if a temple is desecrated, well, you have a bunch of angry spirits on your ass.

Lordran - The entirety of Lordran has appeared over Australia and New Zealand, seemingly a warped world trapped within it's own limits, and depending on where you enter you could be tossed into a number of ancient cities and different centuries. Everything that lies within this twisted world, is simultaneously alive and dead, unless they manage to escape the hollow world by lighting the cinders of the first kiln once more.

Lovecraftian - R'yleh, and various unknown locations.
