I writing to you to today in regards to the decision made July 22nd 2024 where The Disability Adjudication Unit reviewed my disability and found that I meet the programs definition of disability, But did not schedule a review of my disability. My question pertaining to the matter being that i would like a copy of the confirmed diagnosis associated with my Ontario disability support account number in the adjudication units format. I had also wanted to ask about weather or not i could make revisions to my self diagnosis or updates to the self report. My third request is for any and all information relating to doctor abdel found in the internal review. I plan on moving forward with legal action and would love any and all assistance in this regards as I am conducting litigation myself.
