Yes, please refer me It has been over a year and I have not been able to make any progress in legitimizing my business and require assistance.

I have also received a letter in regards to a one year review I have booked an appointment with my doctor that wont be until the 2nd of February I also have some Revisions and corrections to make on my Hospital Medical Chart I have downloaded the form and have been doing my due diligence with documenting any issues. I should be able to have that ready for the review if it is required I am not sure how I am supposed to request the form or paperwork or just provide my documentation after making corrections to my chart. Please let me know how i can coordinate this to work best for you.
