  1. The [Organization] computer network, equipment, and resources (including online and third-party resources) are [Organization] and are provided to support the professional and administrative functions of [Organization]. The use of these computer resources is governed by federal and state law and [Organization] policies.
  2. The use of any Information Technology resource, including computer and e-mail accounts, is subject to the following terms and conditions:

Each account is for the exclusive use of the individual or organization to which it was assigned and users may not allow or facilitate access, including by a proxy or anonymous re-mailer, to [Organization] computer accounts, equipment, or restricted files or systems by others. Authorized users are [Organization] staff and authorized business associates. Divisions and departments may also authorize temporary accounts for use by non-[Organization] personnel strictly for the purpose of conducting [Organization] business.
  4. 2. The use of the account may not violate any policy of [Organization].
  5. 3. The use must not overload the [Organization] computing equipment or systems, or otherwise negatively impact the system's performance.
  6. 4. With the exception of activities related to [Organization], the use must not result in commercial gain or benefit to the users and cannot constitute consulting for a business or running a business. Accounts, e-mails, web-pages, or sites may not promote commercial activities or display paid or un-paid advertising not approved by [Organization].
  7. 5. The use may not violate laws or [Organization] policies against discrimination or harassment due to race, sex, religion, disability, age, or other protected status.
  8. 6. The use may not violate state laws or [Organization] policies on the use of [Organization] equipment, resources, or time for political activities.
  9. 7. The use must not involve sending of soliciting chain letters, nor may it involve sending unsolicited bulk mail messages (e.g., "junk mail," or "spam").
  10. 8. The use may not imply or state [Organization] sponsorship or endorsement, nor use [Organization] logos or trademarks without expressed written permission from on officer of [Organization].
  11. 9. The use may not involve unauthorized passwords or identifying data that attempts to circumvent system security or in any way attempts to gain unauthorized access.
  12. 10. All content, web sites or page or collection of electronic material, including e-mails, are subject to the following disclaimer, regardless if posted or included in the material.: "The views, opinions, and conclusions expressed in this page are those of the author or organization and are not necessarily those of [Organization], officers or directors. The content of this page has not been reviewed or approved by [Organization], and the author or organization is solely responsible for its content."
  13. 11. Organization accounts are subject to deactivation and deletion at any time for any reason.
  14. 12. [Organization] may examine electronic information stored on or passing over University equipment or networks, including e-mail, for the following purposes:
  15. a. to ensure security and operating performance of its computer systems and networks
  16. b. to enforce [Organization] policies or compliance with state or federal law where examination is approved in advance by a dean, vice president, or the president, and either there is reasonable suspicion that a law or [Organization] policy has been violated and examination is appropriate to investigate the apparent violation, or examination is necessary to comply with state or federal law. Computer users should have no expectation of privacy in material sent, received, or stored by them on or over [Organization] computing systems or networks.
  17. 13. Use that violates the terms of the account agreement, state or federal law, or any [Organization] policy may result in referral for action under the appropriate disciplinary procedure and the imposition of sanctions which may include suspension or revocation of access privileges in addition to other sanctions.
  18. 14. [Organization] reserves the right to change or update these terms of use at any time for any reason. You agree to assume to responsibility to be aware of these terms at all times. Current copies are available upon request from the administrative offices of [Organization] and are posted on the Autotown Insurance web site.
  19. Indemnification
  20. You agree to indemnify and hold [Organization], its Parents, Subsidiaries, Affiliates, Officers, Partners And Employees, harmless from any claim or demand, including reasonable Attorneys' Fees, made by any Third Party due to or arising out of your use of [Organization] Services, including technological services, use of your account by any Third Party, the violation of the terms of use by you, or the infringement by you, or any Third Party using your account, of any intellectual property or other right of any person or entity.

By signing this form you agree to the terms of use, and acknowledge that you understand the terms of use for [Organization] Information Technology resources, and that violation may result in suspension of privileges, or other disciplinary action up to and including expulsion. Also, you acknowledge that [Organization] will report any illegal activity to the appropriate agency if discovered.
