- You picked Udina for councilor
- You killed Wrex
- You killed the Rachni Queen.
- You romanced Liara, started romancing Thane, then after being confronted by Liara, romanced her again, and then went right back to Thane.
- You complete Mordin's Loyalty mission and kill Maelon, then make sure to get rid of all data related to cures for the genophage, and then get Mordin killed in the suicide mission.
- You complete Tali's loyalty mission by getting her exiled from the Migrant Fleet, tell the Quarians to go to war with the Geth, then get Tali killed in the Suicide Mission.
- Keep Grunt sealed in the tank
- Do not reactivate Legion, give it to Cerberus.
- Do Kasumi's loyalty mission, but let her keep the gray-box with the bad information inside it... make sure she dies in the Suicide Mission.
- When you do Samara's loyalty mission, kill her and recruit Morinth
- Do not get the important ship upgrades or do other loyalty missions to make sure that some squad-mates die at the start of the suicide mission.
- Do not go through the Omega 4 Relay right away... let your entire Normandy crew be melted down by the Collectors, if Chakwas survived, make sure you send her back to the Normandy alone so she also dies.
- During the rest of the suicide mission, make sure everyone is dead except for the following three... Morinth, Thane, and Zaeed.
- When the Illusive Man makes the request to not destroy the Collector Base, comply, and don't blow it up so Cerberus can "salvage" it afterwards.
- Do Zaeed's loyalty mission AFTER you finish the suicide mission. You can actually kill him during his mission this way. Kill him.

If you did all of these things correctly, the Normandy Crew should consist of You, EDI, Joker, Thane, Morinth, and nobody else. That's a really bad commander, a computer-lady, a crippled pilot, a dying man, and a sexual predator. Good luck in ME3!
