Act 2 ends with The MC and Sara fully reconciling and re-consummating their relationship. After they're done, bro bursts in and tells them that he just barely escaped from a wholesale military slaughter of the people in the church. He thinks the three of you should flee the city. As the trio are fleeing on foot, though, they're mobbed by zombies who were drawn to the area by the military's gunfire.

If the player has paid more attention to the bro than to Sara during the game, Sara gets bitten by a zombie. If the player paid more attention to Sara, the bro gets bitten.

Act 3-1 (Sara bitten)
The MC and his bro take cover at Maya's house and ask her to tend to Sara's wounds. The MC and the bro debate what they should do next. The bro begins to softly suggest that Sara might die, but MC won't accept it.

During the conversation, the bro mentions that he saw the military rounding up zombies and putting them into trucks. The MC decides that the army must have a base of operations somewhere, and that there must be scientists there working on a cure. He asks the bro to accompany him on a mission to find such a cure. The bro thinks it's suicide, but he tags along anyway because he doesn't want to see the MC go alone.

MC is unfocused and gung-ho, but the bro reminds him that they don't even know where this supposed army HQ is. The bro suggests visiting the church for clues. The two arrive and look through the church, where they stumble across Naomi cradling Elly's corpse in a rectory. They ask her to join them, but Naomi says she has nothing left to fight for. She's also enraged at the bro for fleeing when Elly got killed. The bro tells her that joining them might be her chance for revenge, and Naomi finally decides to come along.

She takes them to a hidden room in the church where she's stored a huge arsenal of weapons from the police precinct's weapons cache. She also tells them that the army is likely using an old airfield north of the city as their base. They take some guns, get in Naomi's cruiser and start driving.

They arrive at the airfield and find themselves in the midst of a humungous military operation. Naomi leads the other two in stealthily getting past the fences and guards.

Inside one of the hangars, the three see huge pens full of zombies. Naomi snaps the neck of a guard on duty at the hangar and puts on his uniform. She acts as a forward scout for the MC and the bro, moving towards the base itself and killing two more guards, bringing the MC and the bro their uniforms so that they can infiltrate the base.

Naomi gives them pointers on blending in, and says that they're going to have to hide in plain sight if they're going to make it to the laboratories inside the base itself. The MC and the Bro tell her to lead the way, but she refuses, telling them that she has to stay back in case things go wrong.

Naomi gives MC and the bro walkie-talkies taken from two of the guards she killed and tunes them to the frequency of her police radio, which is separate from the channel used by the army. She tells them to radio her if their cover gets blown. They're skeptical of her plan, but she insists she'll have them covered.

Inside the base, the MC and the bro have several unsettling encounters with military personnel, but slowly work their way deeper and deeper into the sprawling complex. But when the MC tries to use a swipe card to access a room that the card doesn't have clearance for, an officer quickly confronts them and begins to suspect that they're infiltrators.

Panicking, the two flee. All the personnel on base are mobilized in pursuit of them. Out of options, the MC radios in to Naomi but receives no response.

Soon, however, alarms begin to go off and a voice over the intercom system says that there has been a containment breach -- the pens full of zombies have been opened. Bro puts two and two together and tells the MC that Naomi released the zombies to buy the two of them some time. We cut to a scene that confirms this as Naomi gets eaten by the zombies. She smiles and thinks about how she really did get her revenge on the army.

The MC and the bro work their way through the complex some more. Soldiers are no longer pursuing them, and the few soldiers they do see are rushing to the airfield to battle the zombies, and don't pay any attention to them. After some searching, the two finally find a laboratory where dozens of men in lab coats are working. The MC grabs one and puts a gun to his head, demanding to speak to the chief scientist.

The chief scientist steps forward and has a long conversation with the MC. The MC demands to know why the outbreak started, but all he gets in response is a shrug and "your guess is as good as mine." The MC refuses to believe that the scientists had no hand in it. He demands to be given access to the cure. The lead scientist insists there is no cure. Enraged, the MC kills the lab worker he has hostage and says he'll kill more of them if they don't give him what he wants.

The lead scientist continues to insist that they don't have the cure -- and that they're not looking for one. He says that the disease is incurable, and that once a person has been bitten, they're doomed. The MC collapses into hysterics. The bro asks the lead scientist what the lab is for if they're not working on a cure. The scientist says they're experimenting on the zombies to see if there's a biological agent they can drop over the city to kill them without using explosives.

BAD END
The MC snaps, shoots the lead scientist, and then tells the bro to tell Sara that he's sorry he couldn't save her. He then shoots himself in the head.

GOOD END
The MC snaps and shoots the lead scientist. He tells the bro to tell Sara that he's sorry he couldn't save her and puts the gun to his own head. As the other scientists flee the lab, the bro counters with a heartfelt speech about how he can't bear to lose another friend today, especially not the MC, who matters more to him than anyone else in the world. He convinces the MC to escape the base with him. The two make it out alive, go back to the cruiser, and drive toward the city again.

They get back to Maya's apartment. Sara is in the final stages of the sickness, and is clearly dying. The MC tearfully apologizes for failing her, and she forgives him. She asks him to kill her so she doesn't become a zombie. He does.

We get a long narration from the MC about how after Sara's death, he started living at Maya's house with the bro. He says that it took a long time to accept that Sara was gone, but that the bro and Maya's support helped him. We end on a shot of the MC standing on Maya's balcony, looking at a sunset, the bro with his hand on the MC's shoulder. The MC thinks about how hard life is now, but how grateful he is to have a friend who would go to the ends of the Earth for him. He says that Maya and the bro are his new family now.
