I think it's 100% completed/accurate but please feel free to double check, especially the Rhea/Petrus sequences. I don't have the memory to double check it anymore.

This is formatted to be best viewed in notepad with Word Wrap OFF. This could be converted to a spreadsheet for even better formatting.

	NPC - Solaire - Talk and say Yes

	BOSS - Gargoyles

Catacombs
	NPC - Patches - Say No, then speak again after event and say Yes or No and don't attack (souls reward for no)
	BOSS - Pinwheel - Summon Paladin Leeroy from one of the cliffs you need to drop onto

Lower Undead Burg
	BOSS - Capra Demon

Firelink Shrine
	NPC - Petrus - Talk
	NPC - Reah and Guards - Exhaust dialogue - Reload until they're gone and again until Petrus returns
	NPC - Petrus - Talk
	NPC - Lautrec - He will offer a tip at the cost of 100 souls per soul level. You must have the souls the first time.

Depths
	NPC - Kirk - Get invaded
	BOSS - Gaping Dragon - Summon Solaire or Lautrec - Although they will make the fight harder and this isn't needed for anything.

Blighttown
	NPC - Get invaded by Maneater Mildred in human form near the swamp-level bonfire
	NPC - Quelaana - She only appears if you have a +10 Pyromancy Flame. Near one of the pillars in front of the boss' tunnel.
	BOSS - Quelaag - Summon Maneater Mildred
	BOSS - Ceaseless Discharge - Now or anytime before O&S

Sen's Fortress
	NPC - Siegmeyer - Redirect balls
	NPC - Big Hat Logan - Use the balls to hit the wall where a resting Man-Serpant was leaning against. Free from cage. He will sell spells at Firelink until you place the Lordvessel.
	BOSS - Iron Golem - summon Black Iron Tarkus

ANYTIME BEFORE O&S
	BOSS - Moonlight Butterfly with summoned Beatrice - Necessary if you want her for the Four Kings
	BOSS - Sif
	BOSS - Ceaseless Discharge

Anor Londo
	NPC - Solaire - Talk
	NPC - Siegmeyer - Kill knights in nearby room, talk to him again
	NPC - Lautrec - Use the Black Eye Orb in big room leading to O&S. Use the Firekeeper Soul at Firelink and talk to Anastacia.
	BOSS - O&S - Summon Solaire - Do not talk to Frampt or use Lordvessel

Firelink Shrine
	NPC - Siegmeyer - Talk, get miracle

Blighttown
	NPC - Siegmeyer - Near Giant Leeches and Server - Give 3 Purple Moss Clumps

New Londo Ruins
	BOSS - Four Kings - Summon Beatrice - Her corpse can later be found in Valley of Drakes near the Lew Londo Ruins shortcut
	NPC - Kaathe - Make the choice of joining Kaathe or Frampt. You can talk to him and get brought to the altar and then Homeward Bone or Homeward out to get all dialogue and still join his covenant without picking him.

Undead Parish
	NPC - Solaire - Talk - At the Altar of Sunlight (optional but explanatory)

The Duke's Archives
	NPC - Big Hat Logan - He's in the big cell at floor level, you need the Archive Tower Giant Cell Key you'll find shortly later. He moves to the book room near the tall ladder and lever shortcut and sells spells unique to his time here. After buying everything, continue talking. Reloading (returning after warp might be necessary) and you can find him in Seath's original room. Return to old room for some equipment.
	NPC - Sieglinde - In a Golden Crystal Golem in field before Crystal Cave after Siegmeyer leaves Firelink Shrine
	BOSS - Seath

Firelink Shrine
	NPC - Sieglinde - Tell her you have seen her father

Demon Ruins
	NPC - Kirk - Get invaded - Ceaseless must be dead and Lordvessel must be placed
	BOSS - Demon Firesage
	BOSS - Centipede Demon - Summon Solaire
	NPC - Solaire - Continue through tunnel to find him near bonfire and talk. DO NOT ENTER IZALITH. Go back the way you came.

Quelaag's Domain
	Become level 2 with the Chaos Servants. You need to feed her 30 soft humanity. They are quickest to farm from the skeleton babies right before the Nito fight. Careful, they drop and disappear within a second or two.
	NPC - The Fair Lady - You can speak with her only if you equip the Old Witch's Ring. You can get this by trading the Sunlight Maggot you get in the following steps to Snuggly the Crow.

Demon Ruins
	NPC - Solaire - When you are +2 with the Chaos Servants, you can open the door in the overgrown tunnel to the right of the entrance of Demon Firesage's boss room. Kill the red-eyed bug.
	NPC - Solaire - Return to this door after reaching Lost Izalith to talk with him

Lost Izalith
	NPC - Kirk - Get invaded - Near boss' fog gate
	NPC - Siegmeyer - He rushes down to kill the Chaos Eaters. Kill all but 1 with ranged. He must survive with 50% health or more health for final event
	BOSS - Bed of Chaos

Firelink Shrine
	NPC - Sieglinde - Talk

Undead Asylum Revisited
	You can do this anytime but this is a timely time
	BOSS - Stray Demon
	ITEM - Get the Peculiar doll found in your original cell, and trade stuff to Snuggly if you want.

Tomb of the Giants
	NPC - Patches - After event, say Yes or No and don't attack (humanity reward for No)
	NPC - Rhea - Talk, do as she says, Talk
	NPC - Paladin Leeroy - Get invaded in the well-lit cave.
	BOSS - Gravelord Nito

Firelink Shrine
	NPC - Petrus - Talk - Kill

Undead Parish
	NPC - Rhea - In the parish selling miracles. If you buy them all, she will later be found hollowed in a cell in Duke's Archives

Kiln of the First Flame
	BOSS - Gwyn, Lord of Cinder - Summon Solaire

	NPC - Xanthous King, Jeremiah - Get invaded near the impaled corpses
