Dear Sir/Madam,

I request that you provide any and all CCTV/bodyworn video you have recorded of me. I was recorded by your staff outside [Holles Street Maternity Hospital/Rotunda/etc] on [18/06/2018] between [1 HOUR TIME SLOT] on 6 of your cameras.

Note that this is a request under the General Data Protection Regulation (GDPR). You must comply within 30 days, you must redact/blur out all faces and vehicle registration plates you may have captured of other members of the public. Non compliance will result in a formal complaint to the Data Protection Commissioner and fines of up to €20 million Euro.

I further request that, on completion of this work and supply of the video, you then delete all details you hold on me, including this email.
