This form allows to register a team for the "osu!mania International Tournament" from 18 October to 16 November 2014.

Substitutes:

- The captain is also a player of the team.

- The tournament is by team. Each country has only one team and each team will be composed of 5 players with 3 substitutes. Those 5 players have one k assigned and each team should find his 4k, 5k, 6k, 7k and 8k players.

- Substitutes will be versatile and could replace the k that they want, He can't play 2 different key modes in the same round.

- The replacement of a member of the team will have to be announced one hour before the beginning of the round. Otherwise the team will be disqualified of the tournament.
