- Ghastly Appearance: Any humanoid creature that starts its turn within 10 feet of the ghoul wife and can see her must make a DC 12 Wisdom saving throw. On a failure, the creature is frightened until the end of its next turn.
- Actions:
- Bite: Melee Weapon Attack: +4 to hit, reach 5 ft., one creature. Hit: 9 (2d6 + 2) piercing damage. If the target is a creature other than an undead, it must succeed on a DC 12 Constitution saving throw or be paralyzed for 1 minute. The target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.
- Claws: Melee Weapon Attack: +4 to hit, reach 5 ft., one creature. Hit: 9 (2d6 + 2) slashing damage.
- Banshee Scream (1/day): The ghoul wife releases a terrifying scream in a 30-foot cone. Each creature in that area must make a DC 12 Constitution saving throw, taking 14 (4d6) psychic damage on a failed save, or half as much damage on a successful one. A creature that fails its saving throw is also frightened for 1 minute. The frightened creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. Once the ghoul wife uses this ability, it can't use it again until it completes a short or long rest.
