After completing Duty, Honor, Country I am not able to start the next quest A Matter of Tradition. This is because I exited the room and once you have exited Duty, Honor, Country you are not able to re-enter the Heart of the Sworn room via Bartholomew.
During the quest "Duty, Honor, Country" I entered the Heart of the Sworn room by talking to a Sultansworn Elite. I finished the quest, at which point the next main story quest A Matter of Tradition became available from Owyne. I exited the building without taking on the quest, went outside and logged off. When I logged back in I noticed that I could no longer access the next quest. It doesn't appear in my journal or my recommendations list and the Sultansworn Elite I spoke to earlier is now gone. I can't access the Heart of the Sworn and I'm completely locked out of the main questline as a result.
Checked recommended duty in Ul'dah and Thanalan areas. Then I talked to Momodi (no quest) and Bartholomew (no quest). I do not have access any more to the Heart of the Sworn room that is through the Royal Promenade in the Steps of Thal in Ul'dah.

Note: this is not caused by running the game through Steam shortcut. It happens when I use the official FFXIV direct launch executable/shortcut.
