  19. We have discovered that between April 17 and April 19, 2011,
  20. certain PlayStation Network and Qriocity service user account
  21. information was compromised. As a result of what we have found
  22. to date, we have:
  23.  
  24. 1) Temporarily turned off PlayStation Network and Qriocity services;
  25.  
  26. 2) Engaged an outside, recognized security firm to conduct a full and
  27. complete investigation into what happened; and
  28.  
  29. 3) Quickly taken steps to enhance security and strengthen our network
  30. infrastructure by rebuilding our system to provide you with greater
  31. protection of your personal information.
  32.  
  33. We greatly appreciate your patience, understanding and goodwill as
  34. we do whatever it takes to resolve these issues as quickly and efficiently
  35. as practicable.
  36.  
  37. Although we are still investigating the details of this incident, we
  38. believe that an unauthorized person has obtained the following information
  39. that you provided: name, address (city, state, zip), country, email address,
  40. birthdate, PlayStation Network/Qriocity password and login, and handle/PSN
  41. online ID. It is also possible that your profile data, including purchase
  42. history and billing address (city, state, zip), and your PlayStation
  43. Network/Qriocity password security answers may have been obtained.
  44. If you have authorized a sub-account for your dependent, the same
  45. data with respect to your dependent may have been obtained.
  46. While there is no evidence at this time that credit card data was
  47. taken, we cannot rule out the possibility. If you have provided your
  48. credit card data through PlayStation Network or Qriocity, out of an
  49. abundance of caution we are advising you that your credit card number
  50. (excluding security code) and expiration date may have been obtained.

  52. For your security, we encourage you to be especially aware of email,
  53. telephone and postal mail scams that ask for personal or sensitive
  54. information. Sony will not contact you in any way, including by email,
  55. asking for your credit card number, social security number or other
  56. personally identifiable information. If you are asked for this information,
  57. you can be confident Sony is not the entity asking. When the PlayStation Network
  58. and Qriocity services are fully restored, we strongly recommend that you
  59. log on and change your password. Additionally, if you use your PlayStation
  60. Network or Qriocity user name or password for other unrelated services or
  61. accounts, we strongly recommend that you change them, as well.

  63. Under Massachusetts law, you have the right to obtain any police report
  64. filed in regard to this incident. If you are the victim of identity theft,
  65. you also have the right to file a police report and obtain a copy of it.
  66.  
  67. Massachusetts law also allows consumers to place a security freeze on their
  68. credit reports. A security freeze prohibits a credit reporting agency from
  69. releasing any information from a consumer's credit report without written
  70. authorization. However, please be aware that placing a security freeze on
  71. your credit report may delay, interfere with, or prevent the timely approval
  72. of any requests you make for new loans, credit mortgages, employment, housing
  73. or other services.

  75. If you have been a victim of identity theft, and you provide the credit
  76. reporting agency with a valid police report, it cannot charge you to place,
  77. lift or remove a security freeze. In all other cases, a credit reporting
  78. agency may charge you up to $5.00 each to place, temporarily lift, or
  79. permanently remove a security freeze.
  80.  
  81. To place a security freeze on your credit report, you must send a written
  82. request to each of the three major consumer reporting agencies:
  83. Equifax (www.equifax.com); Experian (www.experian.com); and
  84. TransUnion (www.transunion.com) by regular, certified or overnight
  85. mail at the addresses below:

  100. In order to request a security freeze, you will need to provide the following information:
  101.  
  102. 1. Your full name (including middle initial as well as Jr., Sr., II, III, etc.);
  103. 2. Social Security Number;
  104. 3. Date of birth;
  105. 4. If you have moved in the past five (5) years, provide the addresses
  106. where you have lived over the prior five (5) years;
  107. 5. Proof of current address such as a current utility bill or telephone bill;
  108. 6. A legible photocopy of a government-issued identification card
  109. (state driver's license or ID card, military identification, etc.)
  110. 7. If you are a victim of identity theft, include a copy of either the
  111. police report, investigative report, or complaint to a law enforcement agency
  112. concerning identity theft;
  113. 8. If you are not a victim of identity theft, include payment by check,
  114. money order, or credit card (Visa, MasterCard, American Express or
  115. Discover only). Do not send cash through the mail.

  117. The credit reporting agencies have three (3) business days after receiving
  118. your request to place a security freeze on your credit report. The credit
  119. bureaus must also send written confirmation to you within five (5) business
  120. days and provide you with a unique personal identification number (PIN)
  121. or password, or both, that can be used by you to authorize the removal
  122. or lifting of the security freeze.

  124. To lift the security freeze in order to allow a specific entity or individual
  125. access to your credit report, you must call or send a written request to the
  126. credit reporting agencies by mail and include proper identification (name, address,
  127. and social security number) and the PIN number or password provided to
  128. you when you placed the security freeze as well as the identities of those
  129. entities or individuals you would like to receive your credit report or the
  130. specific period of time you want the credit report available. The
  131. credit reporting agencies have three (3) business days after receiving
  132. your request to lift the security freeze for those identified entities or
  133. for the specified period of time.

  135. To remove the security freeze, you must send a written request to each
  136. of the three credit bureaus by mail and include proper identification
  137. (name, address, and social security number) and the PIN number or
  138. password provided to you when you placed the security freeze.
  139. The credit bureaus have three (3) business days after receiving your
  140. request to remove the security freeze.
  141.
