The purpose of this questionnaire is to ensure that all candidates and current employees are fit for the roles they have applied for or are engaged in.
  3. This information will be treated with sensitivity and will only be shared in confidence with relevant stakeholders for recruitment purposes only.

  5. Previous claims and injuries will not prevent you from employment as long as we are satisfied on a reasonable basis that you can fill the inherent requirements of the role.

Section 79 of the Workers Compensation and Injury Management Act 1981 gives the Arbitrator discretion to refuse to award compensation which otherwise would be payable; where it is proved that the worker has, at the time of seeking or entering employment in respect of which he/she claims compensation for an injury, willfully and falsely represented himself/herself as not having previously suffered from the injury.
  8. Failure to accurately and completely provide the information requested in the report may lead to disciplinary action up to and including termination of employment.
