I've installed PHPmyAdmin on both servers now, and configured the DNS. You'll find the entry URLs below. They're protected by HTTP basic auth, for which the credentials are below, as well.

PRODUCTION:

phpmyadmin.web1.prod.interplex.anchor.net.au

User: interplex
Pass: olive4beauty8lest

STAGING:

phpmyadmin.web1.stg.interplex.anchor.net.au

User: interplexstg
Pass: grief8quid+even

Afterwards you can log into your databases with the usual MySQL credentials.

I've checked your PHP and it's working. I've also checked the enabled mods for mod_rewrite, and it's enabled in Apache. I'm not sure at the moment where you're seeing this. Could you please send me an example, or a way to reproduce the issue? Thanks!
