EV Instructions

1.	Log into your MyHSF account to access your EV Form.

Print the EV Form
Follow the instructions provided within
Complete, sign, and date the Release of Information section
Have your Registrar complete the Enrollment Verification section
Scan and upload the completed Enrollment Verification Form by the above deadline
2.	Please Note: Your EV Form must be dated at least one day after your first day of class.

3.	Once you have submitted your Enrollment Verification Form, you will receive instructions on how to complete the final step prior to disbursement of your scholarship funds.
