246. Part VII: Scenes (/16)
247.
248. Please take your time on these questions and provide as much detail as possible, using all of your knowledge gained thus far to fully explain how you would progress through these trauma scenes. (By every step this includes scene assessment, DRCAB etc)
249.
250.
30) You have arrived to a trauma scene and the patient is unconscious and bleeding from a gunshot wound to the shoulder. Explain the full process you would take in as much detail as possible. (9)
251.
252. Well, first of all, I’d go and park the ambulance correctly and grab my BLS/ALS Kit. Then, I would check DR CAB. I will see the surrounding and how it is. If it’s crowded with cars and people, I’d contact the Police Department or the Sheriff Department for assistance and a road block. As well as protection for myself from the other citizens. Now that I got my protection, I’m ready to start. I will start by checking his vitals, pulse and respiratory rate. I will then grind my hands against his chest and see if he’s responsive or no, which will be no because he’s unconscious. I will then open the kit and reach for the trauma scissors and cut the fabric around the GSW, exposing it in order to work properly. If the bullet is still inside, I wouldn’t try to remove it a single inch. I would then grab the trauma dressing and place it over the wound with great pressure. After that, I may take a look on his body suspecting any other wounds. If there aren’t, I’d call for someone from the PD or SD to help me getting the patient on the stretcher while sliding it out of the ambulance. Finally, I would transport the patient immediately to All Saints General Hospital for further treatment.
253.
254. 31) You have arrived to a trauma scene and the patient is unconscious in the middle of the road. A severe compound fracture is visible on the patient's left leg. Explain the full process you would take in as much detail as possible. (9)
255.
256. First of all, I can’t leave the patient like this, in the middle of the road. A car or so may run him over. So I will call Police Department and/or Sheriff’s Department for a roadblock. In the meanwhile, I will somehow block the way infront of him until they arrive. I’ll immediately sprint to the patient with my BLS/ALS kit checking for the vitals. Before anything, I should control the bleeding using dressings. After controlling it, I will loosely cover the bone with gauze pads. I will then ask for assistance from the PD/SD to help me get the patient on the stretcher. As soon as the patient is loaded in the ambulance, I’ll instantly drive to All Saints General Hospital for a better and more accurate treatment.
