1. Excel file containing ALL of the following information pertaining to your stock/security sale transaction: (1) Description (2) Purchase Dates (3) Purchase Price (4) Sale Date (5) Sale Price. Please note, we will need the Excel file in addition to any PDF or paper broker statement you may have provided. You should be able to obtain this information from your broker or your broker's company website and for items reported on a U.S. 1099-B a copy of your 1099-B and Realized Gain / Loss statement (preferably downloaded directly from your Broker's website - help document attached in the following page). If you send an excel file, it MUST contain all of the information as it is reported on the 1099-B including the information found in Box 1b Date of Acquisition, Box 2 Stocks, bonds, etc., Box 3 Cost Basis, Box 4 withheld, Box 5 Wash sale loss disallowed, Box 6 Check if a noncovered security, Box 8 Type of gain or loss and Box 9 Description.
