- Subj: PERMANENT CHANGE OF STATION
- 1. Delivered. Effective [retracted], you will stand detached from your present station and duties. You will proceed and report in to the Commanding Officer, [retracted] on [retracted] for duty no later than [retracted]
- 2. You are authorized 15 days leave and 2 days travel time via government transportation (GTR). Upon completion of the above leave, your leave balance will be 0 days.
- 3. Should an emergency arise and you determine that more leave is required; you must contact your new command in sufficient time for a decision to be made. Your request must be include the reason for additional leave, the number of days required, a leave address and telephone number where you can be contacted, and your leave balance after the additional leave.
- 4. If acute, non-emergency medical care is required for you or your family members call [retracted] to obtain guidance and to receive the required authorization. If emergency care is needed, get care immediately, then contact the number listed above once care is received.
- 5. You have provided your next of kin’s name, address and telephone to be notified in case of an emergency as: [retracted]. You have given your leave address and telephone number as: SAME AS ABOVE. Any change of leave address shall be reported to the commanding officer of your new duty station.
- Receiving Endorsement
- 1. I received these orders at First Marine Division, Camp Pendleton, CA at [retracted] on [retracted]. I understand that I am to proceed and report no later than [retracted], to the Commanding Officer, [retracted], for duty. I have in my possession my MR, DR, and SRB.
- 2. I understand that I am not to execute travel under these orders prior to the time and date stated in paragraph one of these orders.
